The film commences with a chilling news broadcast, casting a somber tone over the desolate landscape that was once Lake Victoria. A year has passed since the gruesome attack by prehistoric piranhas, leaving behind a barren lake devoid of life and a town crippled by the loss of its primary source of income: tourism. The once-thriving beaches now lie dry and lifeless, as if mourning the demise of their former vitality. No explanation is offered regarding the fates of the original survivors, leaving only the haunting silence to fill the void.

Meanwhile, at a nearby lake, Clayton (Gary Busey) and Mo venture into the water in an attempt to recover the body of a deceased cow. As they wade through the depths, it becomes clear that the cow's decomposition has led to a peculiar phenomenon: eggs are hatching within its carcass, causing the metabolism to force their emergence through a most unexpected means – the cow's rear end. The piranhas, oblivious to the danger, swarm onto the two farmers, Mo meeting his demise as he disappears beneath the crimson-stained surface, leaving only his hat bobbing in the blood-red waters behind him. Clayton makes one final bid for freedom, launching himself out of the water, but ultimately succumbs to the piranha's relentless assault. In a defiant act, he severs the head from a piranha and spits it towards the camera as the other predators continue to rip his flesh apart beneath the surface.

Back in town, Maddy (Danielle Panabaker), a marine biology student, returns home for the summer to find her waterpark, co-owned with her unscrupulous step-father Chet, on the verge of re-opening under the dubious moniker "Big Wet." Chet's plans include adding an adult-themed section, complete with topless and fully naked women, as well as a camera that broadcasts their genitalia to a live audience. The waterpark is also home to Big Dave, an employee engaged in a rather unconventional tryst with a water inflow pipe. As the night wears on, Maddy finds herself at a party surrounded by old acquaintances, including her policeman ex-boyfriend Kyle (Chris Zylka) and Barry (Matt Bush), who has long harbored unrequited feelings for her since grade school. She also reunites with two close friends, Ashley (Meagan Tandy) and Shelby (Katrina Bowden). Little does she know, the horrors that lurk beneath the surface of Lake Victoria are about to converge with the seedy underbelly of her own waterpark.

As Shelby (Jean-Luc Bilodeau) and her partner embark on a carefree night by the lake, the tranquility is shattered when a sinister presence invades her most intimate space. The unwelcome visitor sparks a chain of events that will plunge two couples into a descent into terror. Unbeknownst to Ashley and Travis (Paul James Jordan), their evening of passion in their van is about to take a deadly turn. As they indulge in intimacy, the vehicle's handbrake malfunctions, sending it careening into the lake. Panic sets in as the water rises, and the couple frantically searches for the key to release themselves from the handcuffs that bind them.

Ashley, desperate to save herself and Travis, makes a perilous decision: she'll swim for help. Little does she know, this valiant effort will prove to be her downfall. As she struggles to reach safety, Travis meets his gruesome fate at the mercy of the ravenous piranhas. His mangled hand, still cuffed to the van's pole, is a haunting reminder of the horror that has unfolded. Ashley manages to escape to the rooftop of their doomed vehicle, where she gazes in horror as the back window shatters, releasing a torrent of blood and water. Travis's lifeless body sinks beneath the surface, consumed by the merciless piranhas.

As Ashley sits frozen in terror on the van's roof, her screams echo across the lake, but it is too late. The piranhas have already claimed their next victim, leaving behind only the silent, motionless form of what was once a living being. The aftermath reveals the gruesome fate that has befallen both Ashley and Travis, their bodies reduced to mere lifeless shapes on the lake floor.

As the day unfolds, Maddy (no actor name provided) attempts to comfort Shelby about their missing friends, but their somber mood is abruptly disrupted by a relentless onslaught of piranhas. The aquatic predators emerge from the depths, tearing apart the wooden jetty as if it were tissue paper. Panicked, Shelby leaps onto a floating plank, while Maddy wisely urges her friend back to safety, narrowly avoiding the jaws of a snapping piranha that bursts forth from the water's surface. With lightning-quick reflexes, Maddy seizes the beast and throws it onto the sand, where she pummels it with a rock until its lifeless body lies still. However, just as she's about to confirm the piranha's demise, it unleashes one final, feeble snap before expiring.

Shaken but determined, Maddy, Kyle (no actor name provided), and Barry seek guidance from Mr. Goodman (Christopher Lloyd) at Lake Victoria's pet reserve. He agrees to share his expertise, conducting a chilling experiment with the original captured piranha by placing it alongside a small frog on either side of a sheet of steel. The piranha proves surprisingly resilient, managing to bite through the metal barrier mere moments before Barry intervenes, rescuing the frog from certain doom. Goodman's displeasure is palpable as he condemns Barry for foiling his experiment.

As the trio grapples with the true extent of the crisis, they theorize that the piranhas may be migrating via sewage pipes and underground rivers between lakes. Undaunted by their earlier close calls, Maddy and her companions decide to investigate further. They establish that the piranhas cannot breach the outflow pipes connecting Lake Victoria and the adjacent waterpark.

Maddy takes a bold plunge, diving down into the pipelines to inspect the safety grates. Just as she's satisfied with her findings, a swarm of piranhas appears, attacking her in a frenzy of snapping jaws and thrashing bodies. With a burst of adrenaline, Maddy frantically swims for the shore, miraculously avoiding serious injury as she emerges victorious from the aquatic chaos.

As the piranha writhes uncomfortably within her, Shelby confides in Josh that she's not feeling well, pleading with him to join her in an attempt to alleviate her suffering. Her mortality weighs heavily on her mind, and she confesses her deep-seated fear of departing this world a virgin. In a desperate bid to defy the impending doom, she and Josh engage in a furtive intimacy, only for the piranha's insidious presence to turn their moment of passion into a gruesome ordeal. As Josh struggles to free himself from the razor-toothed predator's deadly grip, he's forced to make a harrowing decision: sacrificing his own anatomy with a makeshift scalpel to escape the merciless assault.

Meanwhile, Shelby succumbs to her seizure, only to awaken and embark on a frantic search for Josh. Her worst fears are confirmed when she stumbles upon the severed remnants of his penis lying next to the piranha, accompanied by a pool of crimson fluid. As she turns back to face Josh, she's met with his anguished cries and a gruesome sight: her former lover now holding a blood-stained knife, his eyes blazing with anguish.

As their hospital-bound trauma unfolds, a separate narrative thread reveals Kyle's corruption, perpetuating a web of deceit by accepting bribes from Chet, the cunning entrepreneur secretly siphoning water from an underground aquifer to sustain his lucrative water park. In a moment of calculated retribution, Kyle pens a speeding ticket for Chet, only for the latter to dismissively discard it, branding the former as a crooked cop.

As "Big Wet" finally bursts onto the scene, a star-studded cast converges on its grand opening. Among the esteemed guests is Deputy Fallon (Ving Rhames), who miraculously survived his harrowing encounter with the piranhas last year despite sustaining life-altering injuries - the loss of his legs being the most profound. Joining him is Drew Cunningham (Paul Scheer), a former cameraman still grappling with the trauma inflicted by those razor-toothed fish. As Fallon and Andrew attempt to exorcise their deep-seated fears of the water, the former's bravado masks a lingering vulnerability.

Fallon goads Andrew into pushing him into the pool, but the latter hesitates, earning an impassioned plea from his friend not to subject him to the water's mercy. Undeterred, Fallon repeats his demand, only for Andrew to relent once more as Fallon beseeches him not to succumb to his tormentor's whims. A statuesque beauty intervenes, rebuking Andrew for his bullying tactics and Fallon's pitiful attempts to tempt fate.

Just then, David Hasselhoff makes a splashy appearance as a celebrity lifeguard, complete with his signature swagger. A young boy, oblivious to the perils lurking beneath the surface, becomes the unwitting victim of a piranha attack on his leg. His mother downplays the incident, reassuring him that it was just a minor scrape.

The boy approaches Hasselhoff, requesting a band-aid for his wound. The Baywatch icon, mistaking the youngster's admiration for himself, obliges with an autograph - only to discover that the child has no idea who he is. Undeterred, Hasselhoff regales the young fan with tales of his illustrious career, spanning from Knight Rider to SpongeBob Squarepants: The Movie, but still receives a blank stare. Convinced he's found a kindred spirit, Hasselhoff decides to take the boy under his wing, only for the youngster to bid him adieu and re-enter the pool, leaving Hasselhoff to ponder the fleeting nature of fame.

As Maddy delves deeper into the mystery surrounding the park and the underground river, she endeavors to shut down the waterpark, only to be thwarted by Chet and Kyle. However, their malevolent intentions are soon overshadowed by a more sinister force: the piranhas. The ferocious fish make their way to the area, unleashing a brutal assault on the lifeguards and waterpark patrons. Amidst the chaos, Big Dave finds himself in a peculiar predicament when a piranha becomes lodged in his posterior. Meanwhile, the young boy who befriended Hasselhoff earlier is once again attacked by the piranhas. Hasselhoff, alerted to the boy's cries for help, responds with characteristic bluster, deeming him a "stupid little ginger moron." Deputy Fallon takes drastic measures to protect the visitors, attaching a shotgun prosthesis to his legs. With his new limbs, he wades into the water and taunts the piranhas, claiming they're no match for his titanium-enhanced appendages. He then proceeds to blow them out of the water with his shotgun.

As Hasselhoff rescues the young boy, he becomes smugly satisfied that he has finally proven himself a true lifeguard. Instead of expressing gratitude, the boy simply requests to be released. In the midst of this mayhem, Maddy confronts Chet and delivers a powerful punch, blaming him for the catastrophic consequences he helped unleash. Deciding to escape the chaos, she stumbles upon a distraught little girl mourning over the savagely mutilated and lifeless body of her mother. As Maddy attempts to find help for the grieving child, Chet plots his own escape, only to inadvertently run into a young girl who has just lost her mother to the piranhas. Sympathetic towards her plight, he offers her some of the money he had obtained earlier and boards a golf cart. However, in his haste, he accidentally reverses the vehicle and strikes the poor child. Realizing what he has done, Chet frantically attempts to flee, but his attention is diverted by a banner wire that slices through his neck, ending his life. The cart crashes, sending his decapitated head flying into the adult pool, where it lands between the bloodied breasts of a woman before being sent floating in the water, providing sustenance for the piranhas feasting on its flesh. The camera lingers on the gruesome spectacle as Chet's head is consumed by the piranhas.

As the desperate situation unfolds, Maddy implores Barry to take drastic measures: shutting down the pumps and reversing the flow of water from the pools. With a sense of urgency, Barry begins the arduous task of draining the pools, but Maddy, consumed by her mission to rescue those trapped in the water, finds herself caught in the suction. The powerful force pulls her down into the depths of the pool, where she's suddenly face-to-face with an enormous Piranha - its razor-sharp teeth and menacing gaze making it clear that she's become its next meal. Barry, spotting the deadly creature, seizes his trusty trident and plunges it straight into the Piranha's skull, ending its threat in a swift and decisive motion.

Meanwhile, Kyle stands by idly, refusing to intervene as Maddy struggles for her life. Undeterred, Barry takes matters into his own hands - or rather, he takes the initiative with an oversized rock. With all his might, he leaps into the water, determined to save Maddy from the clutches of death. The weight of the rock proves too much for him to handle alone, but he perseveres, ultimately managing to pull her back to the surface.

As Maddy's lifeless body breaks the water's surface, Barry springs into action, performing CPR with a sense of urgency. Miraculously, his efforts pay off as Maddy's eyes flicker open, and she takes a moment to process the gravity of her situation. With a mixture of gratitude and relief, she plants a kiss on Barry's lips, acknowledging him as her hero. As their gazes meet, it's clear that Barry has not only saved Maddy's life but also won her heart - his valiant act transforming him into her very own knight in shining armor.

As Big Dave exacts his revenge by dumping concentrated Chlorine into the pipes, a lit cigarette dangling from his lips, the ensuing explosion decimates the piranha population and sends Barry's litter trident soaring through the air. Meanwhile, Kyle is wracked with guilt over abandoning Maddy to her fate and allowing Chet to siphon water from the lake, prompting him to spring into action and attempt to rescue the lifeless bodies of the bathers. However, his efforts are tragically cut short when a trident comes hurtling towards him, striking true at the last possible moment and ending Kyle's life in an instant. The celebrations that follow are short-lived, as Maddy receives a distressing phone call from Mr. Goodman, informing her that the piranhas have evolved and are now capable of moving on land. As if to underscore this ominous development, the tank containing the original piranhas is found to be breached, its contents seemingly escaped into the nearby shop. The film concludes with a chilling tableau: one such piranha emerging from the pool, its jaws snapping with deadly precision, and then, in a bizarre display of locomotion, hopping or leaping through the air. David, the same boy who had earlier been saved by Hasselhoff after a heart-to-heart conversation, attempts to capture this unsettling sight on his phone, despite his mother's protests. His final words - "Piranhas are slow on land" - prove to be his last, as he turns to take more photos and is instantly decapitated by the piranha's lightning-fast strike. The mother's blood-curdling scream echoes through the air as Hasselhoff nonchalantly responds with a callous remark. The credits roll amidst scenes of chaos and despair, with bathers staring in horror at the carnage and the hopping piranha still on the loose. A post-credits trailer teases a potential sequel, this time featuring Hasselhoff as a lifeguard, his iconic mane flowing in the wind as he surveys the troubled waters ahead.
