Plot Summary


In the sweltering Arkansas summer of 1999, Mandy (a nurse) finds herself trapped in a vicious cycle of overwork and addiction. To fuel her habit, she resorts to stealing medications from patients and harvesting organs for a lucrative black market operation, with help from her partner Karen and cousin Regina. The trio’s illicit activities are orchestrated by the ruthless Nicholas, who demands prompt payment in exchange for his services.

As Mandy begins her 12-hour shift at the hospital, she discreetly hands over a freshly harvested kidney to Regina, who is tasked with delivering it to Nicholas. However, in a moment of carelessness, Regina accidentally switches coolers, leaving the valuable organ behind. The consequences are dire: Nicholas demands that Regina retrieve the kidney or risk losing her own organs. Panicked and desperate, Regina turns to Mandy for assistance.

Mandy’s refusal to help sparks a chain reaction of violence and terror throughout the hospital. Regina, disguising herself as a nurse, strikes fear into the hearts of patients and staff alike by brutally killing one unsuspecting individual with bleach. Believing that Mandy will be able to harvest the dead patient’s organs, Regina is left stunned when she learns that the kidneys are useless due to his dialysis treatment.

The hospital’s calm facade is shattered as police arrive to investigate the murder. Regina becomes increasingly erratic and aggressive, threatening to expose the organ trafficking operation unless Mandy intervenes. Eventually, Mandy agrees to help, instructing Regina to wait outside while she sets her plan in motion. In a desperate attempt to salvage the situation, Regina mistakenly takes the bladder of a passing skateboarder instead of the kidney.

Meanwhile, Mandy uses her position to manipulate patients and harvest their organs with deadly efficiency. She kills another patient by administering an overdose and successfully extracts his kidney. However, when she leaves the organ in a cooler by the vending machine, it vanishes before Regina can collect it.

As chaos ensues, Nicholas’s henchman Mikey wreaks havoc on the hospital, causing terror among staff and patients. In a moment of desperation, Regina proposes kidnapping a comatose patient to provide Nicholas with a bounty of organs. This unfortunate individual happens to be Mandy’s abusive half-brother, who is also struggling with addiction.

Mandy and Regina join forces to take down Mikey, ultimately emerging victorious. The police investigation mistakenly attributes the murders around the hospital to Mikey’s actions. As Regina departs, Mr. Kent suddenly reveals that he has been uncovering the coolers containing organs throughout the night. Exhausted, Mandy retreats to her truck for a well-deserved nap. The film concludes with Mandy re-entering the hospital for another shift, unaware of the impending doom that awaits her in the form of Nicholas’s arrival.
