In the heart of Missouri, a notorious serial killer named Garrick Turrell orchestrates a daring escape from police custody, focusing his ominous intentions on his ex-girlfriend, Sarah. His perilous journey towards her becomes a grim pilgrimage, leaving a bloody trail of innocent victims in his wake, all while an undercurrent of remorse looms over him, threatening to consume his very being. Meanwhile, Sarah, portrayed by Amy Seimetz, is still grappling with the emotional fallout from Garrick’s imprisonment. She finds comfort in her budding relationship with Kevin, a fellow recovering alcoholic, and their bond deepens as they share a passionate night together.

As their connection flourishes, Kevin takes Sarah to a secluded cabin nestled in the woods, where she unknowingly walks into a sinister gathering of fans obsessed with Garrick’s twisted narrative. This group, consisting of three men, has been scheming to exact vengeance on Sarah for her part in Garrick’s arrest. In a horrifying turn of events, they brutally assault her, relishing in their sadistic fantasies as they hang her upside down. Just when it seems like all hope is lost, Garrick appears at the cabin, heralding imminent chaos and violence.

As the harrowing situation unravels, it becomes evident that one of Kevin’s associates has been covertly communicating with Garrick while he was incarcerated, feeding the killer unsettling clues about Sarah’s whereabouts and their malicious intentions. Garrick’s motives for his prison break become disturbingly clear: he seeks to thwart his fanatics from inflicting further harm on the woman he still harbors feelings for, alongside a twisted moral code that compels him to intervene, despite his dark and violent nature.

In a fierce and desperate showdown, Garrick confronts his three adversaries, ultimately overcoming them, but not without sustaining grave injuries himself. As his life begins to slip away, he courageously frees Sarah from her tormentors, granting her the chance to flee into the enveloping darkness, leaving behind a scene of carnage and despair.
