It all starts with a TV newsman reporting on Flight 180 and its survivors, emphasizing that only one individual, Clear Rivers (Ali Larter), has made it through alive. Next, we meet Kimberly (A.J. Cook), who is about to embark on a road trip with her friends. As they hit the road, she drives alongside her girlfriend while two stoners lounge in the backseat. During their drive, her father calls to alert her about the low transmission fluid, advising her to check it out. She acknowledges the warning but her friend insists they should keep driving despite a warning light illuminating on the dashboard.

As they continue, Kimberly encounters a chaotic scene involving a bus filled with rowdy football players, a jock, a cyclist, a mother and son, a truck loaded with logs, a drunken driver, a stoner, and a cop. In a sudden chain of events triggered by the cop spilling his coffee, catastrophe strikes. Logs tumble from the truck, wreaking havoc as they smash through windows, causing the stoner’s car to flip, the cyclist to collide with a motorcycle, and leading to explosions involving the mother and son. Just as tragedy seems to settle, Kimberly’s vehicle flips as well, and she thinks things are alright until she witnesses the fiery demise of the jock’s car. Just then, she wakes up to realize it was nothing but a nightmare.

Pulling over to gather her thoughts, Kimberly blocks traffic and explains what she saw to a nearby cop, Thomas (Michael Landes). Initially skeptical, he believes her only when the truck zooms by, resulting in a series of explosions that ultimately claim the lives of her friends. Distraught, she is taken to the police station where conversations of coincidences arise, drawing parallels to the fate of Alex from the original film. The group discusses their predicament, pondering if death is coming for them next, and they learn of Clear Rivers, the only survivor from the previous disaster.

The film then shifts to Evan (David Paetkau), a lottery winner, facing a series of unfortunate events, including getting his hand trapped in a garbage disposal.
