In the gritty streets of Dorchester, Boston, private investigators Patrick Kenzie (Casey Affleck) and his partner and girlfriend Angie Gennaro (Michelle Monaghan) are drawn into a desperate plea for help by Helene McCready (Amy Ryan), whose four-year-old daughter Amanda (Madeline O'Brien) has been abducted along with her beloved doll, Mirabelle. Amidst the chaos of a televised appeal, Patrick and Angie are approached by Amanda's aunt Beatrice "Bea" McCready (Amy Madigan) and uncle Lionel (Titus Welliver), who implore them to take the case despite Helene's tumultuous past, which includes a penchant for substance abuse. As they delve deeper into the mystery, the duo discovers that Helene's reliance on drugs has led her down a dangerous path, with her boyfriend Ray entangled in a world of addiction and deception.

Patrick utilizes his network of underworld connections to uncover the truth behind Amanda's disappearance, revealing that Helene and Ray were mere pawns in a larger game orchestrated by Cheese (Edi Gathegi), a ruthless Haitian drug lord. The duo learns that Helene and Ray had stolen $130,000 from Cheese, but their attempts to cover their tracks ultimately led to Ray's gruesome demise at the hands of Cheese's henchmen. With this new information, Patrick and Angie join forces with seasoned detectives Remy Bressant (Ed Harris) and Nick Poole (John Ashton), concocting a plan to locate Amanda, whom they suspect has been taken by Cheese.

As tensions mount, Helene's desperation reaches a boiling point, and she confesses that she had buried the stolen money in her backyard, tearfully entreating Patrick to bring Amanda home safely. With Remy and Nick set to meet with Cheese, exchanging the recovered cash for Amanda's safe return, the stakes are raised, and the clock is ticking for Patrick and Angie to unravel the tangled threads of this desperate case before it's too late.

As Patrick engages in tense negotiations with Cheese, he attempts to broker a deal to secure Amanda's safe return by offering the stolen money in exchange. However, Cheese remains resolute in denying any involvement in the young girl's disappearance. The following day, Captain Jack Doyle presents Patrick with a telling telephone transcript, revealing that Cheese had orchestrated a clandestine exchange at a nearby quarry. As it turns out, Cheese was willing to go to great lengths to reclaim Amanda, even venturing far beyond the city limits. When the botched exchange devolves into chaos, a violent gunfight erupts, and Cheese meets his untimely demise. In the aftermath, it is believed that Amanda fell prey to the quarry's treacherous waters, her beloved doll being recovered by Angie from the depths of the pond.

As Doyle processes the devastating outcome, he is compelled to revisit his own painful past, which had seen him lose a loved one years prior. The public outcry and scrutiny prove too much for him, ultimately prompting an early retirement. Two months later, another tragedy strikes when a seven-year-old boy vanishes in Everett, only to have Patrick receive intelligence suggesting the perpetrator is none other than Corwin Earle, a notorious pedophile sharing a home with two married cocaine addicts.

With Remy and Nick by his side, Patrick gains entry into the suspect's abode, where he uncovers evidence of the abducted child. As they prepare to launch a daring nighttime rescue, chaos erupts as the woman fatally wounds Nick before chasing Patrick deep into Corwin's lair. In a gruesome discovery, Patrick finds the lifeless body of the young boy, and in a moment of cold calculation, he puts an end to Corwin's evil schemes.

The following evening, Remy, still reeling from the events, attempts to ease Patrick's guilt by confiding in him about a past incident where he planted evidence on an abusive husband to help his family escape with Ray's assistance. Patrick is struck by this revelation, recalling that Remy had initially denied any knowledge of Ray.

As the somber atmosphere surrounding Nick's funeral begins to dissipate, Patrick turns his attention to unraveling the tangled web of deceit that has ensnared him and those he cares about. He approaches Officer Devin (Michael K. Williams), a seasoned authority figure, and shares his suspicions regarding Remy's true intentions. Devin's revelation sends shockwaves through Patrick's mind: it appears that Remy and Doyle were well aware of Cheese's stolen money before the fact, and had deliberately manipulated events to stage a fake kidnapping, exploiting Helene in the process.

Patrick's ire is further piqued as Lionel is goaded into meeting him at a local watering hole. The ensuing conversation pieces together the sinister plot: Remy and Doyle had orchestrated a heist, using the cover of a fake kidnapping to pilfer the drug money and teach Helene a lesson. When Patrick discovers the truth, Lionel's admissions confirm his worst fears. The revelation that Bea's desperation led her to involve the media, further ensnaring Patrick in the chaos, only adds to his sense of betrayal.

As if the situation weren't dire enough, Remy appears at the bar, donning a mask and attempting to stage a robbery. But Patrick is no fool; he realizes Remy's true intention – to eliminate those who have discovered his scheme. In a bold move, Patrick yells out that Remy has kidnapped Amanda (Amanda), prompting the bartender to take aim at Remy, striking him twice in the back. The wounded Remy flees, pursued by Patrick up the rooftop of a nearby building, where he meets his untimely demise.

In the aftermath, Patrick is grilled by the police, and it becomes clear that Doyle has been involved all along. A visit to Doyle's home yields an unexpected surprise: Amanda, thought to be in grave danger, is alive and well. Doyle's confession paints a grim picture – he was party to the kidnapping and helped orchestrate the fake exchange to frame Cheese. Patrick is faced with an impossible decision: should he return Amanda to her mother, or keep his promise to Helene? Angie's words of caution weigh heavily on him, but ultimately, Patrick decides to honor his commitment and turns Amanda over to the authorities.

As Doyle and Lionel face the consequences of their actions, Patrick and Angie's relationship falters. The couple's breakup is a poignant reminder that even in the darkest moments, loyalty and duty can be overwhelming forces. Later, Patrick visits Helene as she prepares for a date, only to discover her neglectful parenting has not improved. His willingness to watch Amanda reveals his deep-seated concerns about the young girl's well-being. As he sits with Amanda, he learns that her cherished doll is named Anabelle – a painful reminder that Helene has been absent from her daughter's life in more ways than one.
