As the clock strikes six years since the last terrifying encounter, Michael Myers and his niece Jamie Lloyd find themselves in the clutches of the enigmatic Man in Black and his eerie cult. The dark ritual has taken its toll on the pair, leaving them vulnerable to the sinister forces that surround them.

On Halloween Eve, a desperate Jamie, now fifteen, gives birth to her own child while making a daring escape from Michael's relentless pursuit. In the chaos that ensues, she commandeers a pickup truck and speeds away, with Michael hot on their heels.

Meanwhile, Dr. Loomis, once a stalwart defender against Michael's evil, has hung up his keys as a psychiatrist. However, his peaceful existence is disrupted by a visit from Dr. Terence Wynn, the chief administrator of Smith's Grove Sanitarium, where Michael's dark past began to unfold. As they reminisce about old times, they inadvertently tune in to Jamie's poignant plea for help on the local radio station.

Michael catches up with his niece and brutally dispatches her, only to discover that she had cleverly hidden her child elsewhere. The cycle of violence has begun anew.

In Haddonfield, a new generation of Strodes has taken residence at the infamous Myers house. John and Debra, along with their adult children Kara and Tim, have settled into the old homestead, unaware of the horrors that are to come. Their six-year-old grandson Danny is plagued by ominous visions of the Man in Black, urging him to commit unspeakable acts.

Across the street, Tommy Doyle, a recluse haunted by his own traumatic experiences with Michael, has become obsessed with unraveling the truth behind the killer's motivations. He stumbles upon Jamie's distress call and tracks it down to a bus station, where he finds the infant, whom he names Steven.

As events unfold, Tommy crosses paths with Dr. Loomis and shares his findings about the Strode family. Believing Michael is under the sway of an ancient Druid curse known as Thorn, which drives individuals to slaughter their own families on Halloween night, Tommy takes it upon himself to safeguard Kara and Danny. He whisks them away to the boarding house, where he hopes to find solace and protection from the impending doom.

Michael, meanwhile, returns to his childhood home, unleashing a bloodbath that claims the lives of Debra, John, Tim, and Tim's girlfriend Beth. The stage is set for a terrifying showdown between Michael and those who dare to stand in his way.

As Mrs. Blankenship shares a chilling revelation with Kara, it becomes clear that her young son Danny is being haunted by an otherworldly presence eerily reminiscent of Michael's own supernatural affliction. The enigmatic figure known as the Man in Black, later revealed to be Dr. Wynn, summons Danny to the Myers' residence, prompting Kara to embark on a perilous rescue mission to save her child from Michael's clutches. Upon their return to the boarding house, they're met with an unsettling surprise: Dr. Wynn, flanked by cult members, abducts Kara, Danny, and Steven, whisking them away to Smith's Grove. Kara is confined to a maximum-security ward, while the boys are subjected to sinister experimentation in an operating room.

Meanwhile, Tommy and Loomis converge on the sanitarium, where Loomis confronts Dr. Wynn about his macabre research into the nature of pure evil. The cult leader hints that Smith's Grove staff have been studying the very essence of evil to harness its power, and implies that Steven is a successful result of in vitro fertilization, created using Michael's DNA. Wynn attempts to recruit Loomis, citing their shared experience as witnesses to the horrors within Michael. However, Loomis refuses, only to be knocked unconscious by one of Dr. Wynn's accomplices.

As Kara and Tommy navigate the sanitarium, they're pursued by Michael, who suddenly appears and wreaks havoc on Wynn's cult. The trio manages to escape as Wynn and his followers meet their gruesome demise at the hands of the vengeful Michael. In the aftermath, Tommy frees Kara, and together they rescue the children, fleeing Smith's Grove as Michael gives chase into a laboratory replete with fetuses from Dr. Wynn's failed experiments. Tommy injects Michael with corrosive liquid and pummels him into unconsciousness with a lead pipe.

With their escape secured, Tommy, Kara, and the children depart Smith's Grove, leaving Loomis behind to attend to some unfinished business. The eerie silence is shattered only by the distant scream of Loomis, his fate hanging precariously in the balance. As the scene fades, Michael's mask lies abandoned on the laboratory floor, a haunting reminder of the terror that lurks just beyond the edge of reality.
