Katie, played by Jemma Dallender, is an ambitious model hailing from Missouri, currently working as a waitress in a bustling New York City diner. Eager to refresh her portfolio, she responds to an ad promising free professional photographs. This leads her to an encounter with three Bulgarian siblings: Ivan, portrayed by Joe Absolom, who acts as the photographer, and his brothers, Nikolai (Aleksandar Aleksiev) and Georgy (Yavor Baharoff), who assist him. During the shoot, Georgy develops an infatuation with Katie, but their encounter takes a dark turn when Katie disagrees with Ivan over a topless photo.

In a shocking twist, Georgy later invades Katie’s apartment, where an assault ensues—he binds, gags, and brutally rapes her. In a desperate attempt to intervene, her neighbor Jayson, played by Michael Dixon, arrives but is fatally stabbed, leaving him to witness Katie’s torment. Georgy force-feeds her ketamine, rendering her unconscious. When Katie finally comes to, she finds herself naked, handcuffed to a pipe, and trapped on a filthy mattress in a decrepit basement. Over time, she is subjected to repeated assaults and torture at the hands of the three brothers.

Once she manages to overpower Georgy and escape, Katie discovers she has been abducted to Sofia, Bulgaria. She rushes to a nearby police station where Detective Kiril (Georgi Zlatarev) directs her to Ana (Mary Stockley), who falsely claims to be from a rape crisis center. Instead of helping her reach the American embassy, Ana takes Katie to her home, where Katie is horrifically tormented. It is revealed that Ana is the mother of Nikolai and Georgy, taking grim satisfaction in Katie’s suffering.

Katie experiences further trauma at the hands of Valko (Peter Silverleaf), a friend of the family’s deceased patriarch, who brutalizes her as the torment continues. During a vicious cycle of abuse, she is imprisoned, stripped of her belongings, and buried alive but manages to escape into the sewer system. There, she scavenges just to survive, even stealing from a church. Encountering Father Dimov (Valentine Pelka), who recognizes her plight, she finds temporary refuge and support, though her devastation only deepens.

Eventually, fueled by a desire for vengeance, Katie meticulously plots her revenge against her captors. She sneaks into Ana’s home for money to fund her mission and poisons Georgy’s coffee, drawing him into the sewers where she captures and tortures him.
