In a gripping flashback, we witness a young Josh Lambert, portrayed by Patrick Wilson, struggling with a mysterious woman donning blood-red lipstick who was responsible for Elise’s death at the close of the first film. Concerned for her son, Josh’s mother, Lorraine, reaches out to Elise and her associate, Carl, to address the unsettling issue. They soon discover that young Josh possesses a remarkable ability for astral projection while he sleeps. During an interview with Josh, Elise’s inquiries prompt a series of unsettling reactions from him, intensifying the atmosphere of dread.

Shifting to the present, Renai confronts a police detective regarding Elise’s tragic demise, Heightened by Detective’s findings about potential marks left by Josh, the Lambert family moves in with Lorraine to escape the malevolent forces from their past. However, disturbances quickly return, beginning with eerie piano melodies and the sinister movement of a baby chair. Tension escalates when Lorraine encounters Dalton, who sleepily informs her of a presence lurking behind her. After a series of chilling events, including Renai spotting a disturbing woman in Victorian garb, it becomes apparent that their troubles are far from over.

Driven by concern, Lorraine consults Specks and Tucker, the paranormal investigators who once assisted Elise, hoping they can shed light on these hauntings. Although they are initially unable to help, Lorraine’s mention of Elise sparks a connection with Carl, who effectively reaches out to the deceased medium via word dice. They learn from Elise’s spirit that the vengeful entity they face is Parker Crane’s mother, who has designated herself as ‘Mater Mortis’—Latin for ‘Mother of Death.’

As events unfold, Josh’s behavior becomes increasingly erratic. Disturbing interactions between him and Renai lead her to suspect the malevolent force has returned. Not long after that, the tense atmosphere culminates when they explore the abandoned Our Lady of Angels hospital to uncover the roots of their nightmare. Surrounded by horrific truths about Parker Crane, they are drawn into a conflict with his spirit, leading to tragic outcomes.

Inside the eerie realm of the Further, Carl and Josh navigate the haunting landscape while attempting to reclaim Josh’s soul. They encounter sinister entities and the familiar specter of Elise, who guides them through the chaos. Ultimately, they come face-to-face with the sources of Parker’s pain—his twisted familial ties that birthed his murderous tendencies.

In a harrowing climax, as the group battles to return to safety, they begin to piece together the dark tapestry of their shared nightmares, culminating in a fierce showdown against malevolent spirits. As they successfully return to their own bodies, a new realization dawns: to prevent any future incursions into the Further, Josh, Renai, and their family must undergo hypnosis to erase their memories of astral projection.

Their world seemingly back to normal, the screen transitions to a blinding white light, revealing a new ominous enigma as Specs and Tucker attempt to warn a new victim—they knock on a door, greeted by a family. As they deliver a dark warning concerning the Bride in Black, the lingering presence of the supernatural becomes painfully apparent as Elise appears, aware of a looming terror threatening their peace. A chilling creak echoes from the shadows, leaving viewers with an uneasy sense of dread and anticipation for what lies ahead.
