In 1959, aboard a spacecraft, two aliens are in a frantic race to prevent a potentially dangerous experiment from being released by a reckless crew member. When the third alien, seemingly overtaken by a malevolent force, impulsively shoots a canister into space, it crashes down to Earth. Nearby, a college student takes his date to a secluded spot where they spot a falling star and decide to investigate. Tragically, it lands in the path of an escaped, dangerously insane patient wielding an axe. As the boy’s date is attacked, he discovers the canister from which a small, leech-like creature unexpectedly jumps into his mouth.

Fast forward 28 years, Chris Romero, still reeling from a lost love, finds solace in the friendship of his disabled companion, J.C.. During a fraternity’s pledge week at Corman University, Chris’s eyes land on the beautiful Cynthia Cronenberg, and he finds himself smitten. To win her affections, he resolves to join a fraternity. However, the fraternity leader, who is romantically involved with Cynthia, has no intention of allowing Chris or his friends in. Their initiation calls for the theft of a cadaver from the medical center to showcase on the steps of the Phi Omega Gamma house. In their quest, Chris and J.C. stumble upon a body in a secretive cryogenic storage area, but when it suddenly grabs one of their arms, fear overwhelms them, leading to a frantic escape back to their dorm.

Meanwhile, Detective Ray Cameron, whose past is entwined with the horrific events of 1959, is drawn back into the fray when a break-in is reported at the cryogenics lab. One of the two preserved bodies has vanished—none other than the boy who once unearthed the alien experiment decades ago. This resurrected figure returns to haunt the same sorority house where he picked up his date, and with a dramatic split of his head, an army of leech creatures erupts, slithering across the ground.

The next day, Chris and J.C. face off with the fraternity brothers who wrongfully suspect them of being behind the horrifying incident from the previous night. They survive an intense confrontation, only to find themselves interrogated by the police. The janitor, a witness to their escape, testifies about their behavior, leading to their admission of breaking into the medical center, though they adamantly deny taking the corpse. That night, the deceased attendant rises from his table only to encounter the same janitor, setting the stage for more chaos.

As the eerie events unfold, Cynthia confides in Chris and J.C. about a mysterious dead cat and the strange findings connected to the zombie-like body discovered at the sorority house. Though skeptical of her haunting claims, J.C. observes Cynthia leaning on Chris for comfort, prompting him to step away for a moment. Unfortunately, he encounters the now-possessed janitor, unleashing a new wave of terror as more slugs are released.

Later, as Chris walks back to the sorority house with Cynthia, they cross paths with Ray, who has overheard their chilling narrative. At Ray’s residence, the two share anecdotes about their past loves, where Ray details his heart-wrenching experience of losing a girlfriend in a brutal attack by the same deranged killer. He recounts how he tracked and exterminated the lunatic, burying him on the very site that now houses the sorority. Just then, an urgent call interrupts their conversation— the House Mother has been savagely murdered by the relentless corpse.

As the police corner the revived killer and unleash bullets, their attack proves futile. In a moment of resolve, Ray fires his shotgun, finally decapitating the monster and allowing more of the parasitic creatures to escape.

On the night of the fraternity formal, Chris stumbles upon a tape recording from J.C., revealing that a part of the leech creature has infested his brain and detailing how heat is the key to exterminating them. Racing to the boiler room to rescue his friend, Chris discovers that the mayhem is far from over. He rushes to Ray for assistance, armed with a flamethrower.

As the chaos escalates with Cynthia’s relationship unraveling due to Brad’s possession, Chris and Cynthia team up, brandishing their weapons against the undead outside. Inside, Ray prepares for his final showdown, and as the waves of foes are subdued, Chris sees more slugs heading toward the basement—a place rife with the specimen brains they suspect will orchestrate their doom.

In a high-stakes countdown, Ray ignites gasoline in the basement, prompting a fiery explosion as Chris and Cynthia flee to safety. As they share a poignant kiss while watching the sorority house go up in flames, a chilling finale unfolds—the dog that caused the initial bus accident dramatically reemerges, spewing forth a slug, hinting that the terror might not be over just yet.
