A group of medical residents specializing in pathology concoct a perilous game to determine who can execute the perfect murder. The story revolves around Ted Grey, portrayed by Milo Ventimiglia, who, after graduating at the top of his class, joins one of the nation’s leading Pathology programs. His exceptional talent and determination quickly catch the attention of a select circle of privileged pathology interns. Enamored by his newfound friends, Ted begins to uncover shocking secrets, soon realizing that he has been unwittingly drawn into their sinister after-hours competition at the morgue—a contest to see who can commit an undetectable murder.

As Ted finds himself increasingly entangled in their reckless escapades, the threats become alarmingly real, pushing him to stay a step ahead of the deadly game before he becomes its next target. He reluctantly participates in their twisted pastime, but things take a drastic turn when the group’s leader, Dr. Jake Gallo, learns that Ted is romantically involved with his girlfriend, Dr. Juliette Bath. Fueled by jealousy, Gallo brazenly resorts to murder during their clandestine meetings.

The tension escalates as Ted’s fiancée arrives to stay with him, only for Gallo to react violently to the perceived betrayal, resulting in her death as part of their horrific game. Just as the group prepares to autopsy Dr. Bath, a critical mistake is made when they leave the gas on, leading to a catastrophic explosion initiated by one of the members lighting a meth pipe, miraculously sparing only Ted and Gallo.

Following the explosion, Ted confronts Gallo, who believes he has achieved the perfect murder by killing Ted’s fiancée. With a chilling resolve, Ted compiles the autopsy report of his murdered beloved, only to be ambushed by Gallo. In a gripping showdown, Ted cleverly employs Gallo’s own manipulative rhetoric against him, eventually liberating himself with the help of fellow pathologist, Dr. Ben Stravinsky. In a darkly poetic twist, they exact revenge on Gallo, subjecting him to the same gruesome fate he inflicted upon Ted’s fiancée, leading to a harrowing yet cathartic climax as they begin dissecting a still living Gallo.
