In the blistering heat of Suriname, Clement Molloch, infamously known as The Doctor, takes brutal vengeance on journalist Jorge Hidalgo by subjecting him to a horrifying electric torture session—an event witnessed by officials complicit in his crimes. Meanwhile, in the serene backdrop of the Cayman Islands, Holland, a retired hitman, reluctantly steps back into a world of violence after learning of Jorge’s devastating fate. His resolve is further solidified by the agonizing accounts of Molloch’s victims across the globe, including those from Chile and Asia.

As he prepares for his intense mission, Holland adopts a disguise, masquerading as a family man on vacation. He is accompanied by Jorge’s widow, Rhiana, and her child, Sarah, and they travel to Guatemala, where Molloch operates. Their adventure begins when they meet Max Ortiz, a reliable ally who introduces them to a disreputable cockfighting ring frequented by The Doctor. However, they are followed by a lurking figure that appears to be disabled, but Holland’s instincts sense danger before it strikes.

Later, Max leads them to The Doctor’s fortified compound, providing Holland with a chance to assess the security that could be critical later. Together, Holland and Rhiana wander into town to gather crucial intelligence, with Holland vigilantly protecting her. When a local thug harasses Rhiana, Holland reacts violently, dispatching the assailant in a dramatic confrontation that captures the attention of another of Molloch’s henchmen.

In a bold move, Holland decides to draw The Doctor out by kidnapping his sister, Claire. He sneaks into her hotel room, waiting for her return, only to find her arriving with a female lover. As they share an intimate moment, Holland hides under the bed and, after their exit, he captures a male henchman who enters the room to roll a joint. He swiftly seizes Claire, causing chaos by hanging the thug from a fire hose, enabling their escape.

They take refuge at Max’s ranch, where Holland warns his friend to hide while he plots the next phase to eliminate Molloch. Yet, unbeknownst to Holland, his adversaries are tightening their grip. In a moment of panic, Holland uses a recording of her voice as a diversion, luring Molloch into a trap.
