In the scorching heat of Suriname, Clement Molloch, a mastermind known as "The Doctor", exacts a gruesome revenge on journalist Jorge Hidalgo (Jorge Hidalgo) through an electric torture session, watched with complicity by government officials. Meanwhile, in the tranquil Cayman Islands, Holland, a retired hitman, is approached by an old acquaintance to eliminate The Doctor. Initially hesitant, Holland's resolve hardens upon learning of Jorge's brutal demise and witnessing the harrowing testimonials of Molloch's victims worldwide, including Chile and Asia.

As he gears up for his mission, Holland assumes a disguise, posing as a family man on vacation, and accompanies Jorge's widow, Rhiana, and her daughter Sarah to Guatemala, where The Doctor is based. Their journey begins with an introduction to Max Ortiz, a trusted ally who guides them to a seedy cockfighting ring frequented by Molloch. However, they are tailed by a mysterious figure seemingly crippled, but Holland's keen instincts alert him to the threat before it materializes.

Later, Max takes them to The Doctor's compound, allowing Holland to scope out the security measures for future reference. Holland and Rhiana venture into town, gathering intelligence while he keeps a watchful eye on her well-being. When a local harasses Rhiana, Holland intervenes with brutal force, dispatching the perpetrator in a dramatic display that draws the attention of another henchman.

As tensions escalate, Holland and Rhiana concoct a plan to capture this operative by posing as a trio seeking thrills. They lure him back to their hotel room, where Holland claims his life with calculated precision. Horrified by her husband's brutal methods, Rhiana initially demands they leave the country, but her determination to see The Doctor brought down in person ultimately prevails.

Holland decides to provoke The Doctor into action by kidnapping his sister Claire. He infiltrates her hotel room, waiting patiently for her return. However, she arrives with her female lover, forcing Holland to hide under the bed during their intimate encounter. After they depart, a male henchman enters, rolling a joint; Holland apprehends him as Claire showers. He kidnaps Claire and orchestrates a public spectacle by dangling the henchman from a fire hose tied around his neck, creating chaos that allows them to escape.

With Max's ranch serving as their temporary sanctuary, Holland advises his friend to go into hiding, preparing for the next phase of his mission to eliminate The Doctor.

As Holland remained oblivious to the sinister forces closing in around him, Molloch's underhanded tactics led to Max's capture and brutal interrogation, ultimately extracting from him the location of Holland's hideout. Unbeknownst to Holland, this information was soon exploited by Molloch, who orchestrated a calculated ruse that inadvertently revealed their whereabouts. In response, Holland hastily stowed Claire in the trunk of a vehicle and fled with Rhiana, leaving a trail of chaos in their wake.

The US ambassador, corrupted by Molloch's financial largesse, dispatched a team of assassins to track down the duo. After a high-stakes game of cat-and-mouse, complete with a deadly shootout and frantic chase, the pursuers met their demise, with Claire's lifeless body ultimately succumbing to her confinement in the trunk. Holland and Rhiana then acquired a local farmer's truck, using it as a makeshift shield against the relentless pursuit that followed them to the village of Magdalena.

Upon arriving at this remote outpost, they found solace in the humble abode of a kindly cafe owner, who generously offered them access to his phone. However, their peace was short-lived, as the US ambassador and a ruthless gunman appeared, intent on exacting revenge. Holland swiftly dispatched both adversaries, before seizing the opportunity to confront Molloch directly, demanding that he alone retrieve Claire from an abandoned mine nearby.

Upon arrival at the mine, they discovered a group of disfigured men scavenging for opals amidst the ruins. Initially rebuffed by these wary individuals, Holland and Rhiana were eventually persuaded to abandon their search due to the looming threat posed by Molloch's impending arrival. As fate would have it, this strategic decision proved crucial in the unfolding drama.

Molloch, meanwhile, revealed that he had taken Rhiana's daughter, Sarah, hostage. Panicked by his taunt and desperate to rescue Claire, Holland activated a recording of her voice, which served as a clever decoy. Molloch, predictably lured into the open by this false promise, was met with a chilling reception from the disfigured miners, who had suffered at his hands. As their wrath intensified, they converged on Molloch's vehicle, ultimately dispatching him with brutal efficiency.

With the dust settled, Holland, Rhiana, and Sarah returned to the safety of Holland's island stronghold in the Cayman Islands, their ordeal finally at an end.
