In a twisted exercise in self-identification, Martin decides to recreate the gruesome experiment depicted in The Human Centipede (First Sequence), but with an unsettling twist. Without any medical training or qualms, he assembles a hodgepodge of kitchen gadgets, woodworking tools, and household items into a suitcase, then secures a dilapidated, dark abandoned warehouse as his makeshift laboratory. His goal: to construct not just a three-person centipede, but the “full sequence” of 12 connected individuals, as if attempting to literalize the film’s surreal horrors.

As he embarks on his sinister mission, Martin (clad only in his underwear) makes a series of disturbing phone calls to three actors from the first film, feigning an invitation from Quentin Tarantino’s casting agent to lure them to London. Only one, Ashlynn Yennie (portraying herself), takes the bait. The initial hour of the film is marked by Martin’s brutal attacks, as he mercilessly beats, kidnaps, and renders his victims helpless. His targets include a neighbor, a rental agent, and an expectant couple - all of whom are subjected to his twisted methods.

As Martin claims his victims, he resorts to graphic and gruesome measures to disable them further. He severs the tendons in each person’s legs, rendering them unable to flee. Then, with calculated precision, he knocks out their teeth one by one using a hammer, careful not to allow any of the victims to swallow or choke on their own saliva. Eleven of his victims are subjected to an even more depraved form of mutilation, as Martin slices open their buttocks and uses a staple gun to attach each person’s face to the next person’s anus.

Yennie is spared this particular indignity, instead being chosen to occupy the front of the “human centipede,” allowing her to avoid ingesting feces. As Martin assembles his gruesome creation, he becomes increasingly enthralled by its very existence. His fascination reaches a fever pitch when he decides to administer an excessive dose of laxative to the “centipede,” forcing each individual in the chain to suffer a humiliating and degrading evacuation into the mouth of the person behind them.

Meanwhile, one of Martin’s victims succumbs to their wounds, prompting him to break down in tears. As his depraved fascination reaches new heights, he becomes even more sexually aroused by the desperation and suffering of his “human centipede.” His actions take on a bizarre, almost ritualistic quality as he orchestrates this grotesque spectacle.

Unbeknownst to Martin, one of his victims - an expectant mother - has actually survived her ordeal and regains consciousness. She bursts forth from under a tarp next to a lifeless body, screaming in anguish. In a desperate bid for freedom, she leaps into her car and speeds away. Martin gives chase, throwing himself against the car’s windows in a futile attempt to catch up.

As this chaos unfolds, the expectant mother driving away goes into labor, birthing a child into the footwell of the car. In a macabre twist, she then stomps on the accelerator, inadvertently crushing her newborn’s skull. Martin’s rage is palpable as he watches his own twisted desires being outdone by this unforeseen turn of events.

As Martin’s vendetta against the monstrous “centipede” reaches its bloody crescendo, one of the victims makes a desperate bid for freedom by tearing his face from the person in front, causing the creature to fragment into two distinct entities. Consumed by fury and retribution, Martin returns to the warehouse and unleashes a hail of gunfire upon the remaining eight members of the centipede, racking up a gruesome body count until his weapon runs out of ammunition. With the carnage complete, he takes it upon himself to end their suffering once and for all, methodically slicing the necks of each victim with a kitchen knife.

Meanwhile, Miss Yennie remains the sole survivor, her malevolent spirit refusing to be extinguished even in the face of Martin’s brutal onslaught. As he approaches her to deliver the final blow, she strikes back with a vicious punch to his groin, sending him crashing to the ground in a fit of rage and despair. Seizing the opportunity, Yennie grabs the funnel that had been used to force-feed her earlier and shoves it up Martin’s rectum, before snatching the centipede from its glass cage and inserting it into the wound.

In a shocking turn of events, Yennie then meets her own demise at the hands of Martin, who stabs her in the face with a brutal ferocity. As the dust settles, Martin is left writhing in agony, desperately trying to extract the centipede from his ravaged flesh. The camera pans out to reveal him stumbling out of the warehouse, as the lifeless corpse of Yennie twitches and contorts in the background, raising questions about the true nature of her demise.

The film’s enigmatic conclusion finds Martin, alive and seemingly unscathed, watching a DVD recording of the original “Human Centipede” at his place of work, as the distant wails of a crying baby serve as a haunting reminder that perhaps this was merely a dark fantasy or a precursor to further atrocities.
