The film commences with a poignant montage of tender moments between Skylar "Sky" (character) and Dan, showcasing the joy and intimacy they share. Meanwhile, their wedding celebration unfolds, accompanied by Tommy Covington's observational documentary, a favor requested by Dan. As the festivities reach their peak, an elevator ride for Skylar's Christian parents takes an eerie turn when they suddenly succumb to the inexplicable, their eyes turning milky white. The chaos that ensues is nothing short of apocalyptic: the main floor descends into pandemonium as others fall prey to the same mysterious affliction.

Tommy and Jack flee the scene in search of answers, only to discover a city in ruins, with planes plummeting from the sky and exploding upon impact. In their desperate bid for safety, they're met with an ear-piercing blast reminiscent of a trumpet's call. The air is heavy with foreboding as thunderous claps and lightning illuminate the darkening atmosphere.

As the group regroups outside, Skylar hypothesizes that they're experiencing the biblical phenomenon known as the Rapture – where believers are taken up to heaven, while non-believers are left behind. Tommy, however, dismisses this notion, attributing the events to scientific explanations. Undeterred, Skylar emphasizes the need for a safe haven, as the catastrophes will only intensify with time.

The stormy weather takes a turn for the worse, unleashing monstrous hailstones that pummel the town. Seeking refuge in a library, Tommy encounters Sam, while Skylar finds solace in the Bible. She shares a passage related to the Rapture, specifically The First Trumpet, with Dan, Jack, and Tommy.

As they navigate towards safety, a nearby church becomes their destination. However, just as they near the sanctuary, a malevolent force snatches Skylar from Dan's grasp, leaving her injured on the roadside. With hearts heavy, the group reaches the church, where Pastor Shay welcomes them into the sanctuary, offering respite from the escalating chaos outside.

As Jack steps into the sanctuary of the church, he's reunited with Allison amidst the somber atmosphere. Meanwhile, Dan tries to offer solace to Skylar as she undergoes healing, while Shay provides some much-needed context: they're living in a world ravaged by the Rapture. The gates of the church creak open, allowing more survivors to enter, including a woman on the brink of giving birth. Though the child is born, its lifeless body and lack of vital signs suggest it's stillborn, a grim reminder of the devastation that has unfolded. When Tommy inquires about the cruel fate of these souls, Shay reveals that God has taken the believers' spirits while leaving their mortal bodies behind, a stark reminder of the chasm between faith and mortality.

As they grapple with this existential crisis, a desperate attempt is made to breach the church's defenses, only for a demonic entity to brutally dispatch the would-be intruder, shattering a window in the process. In a poignant gesture, Sam records Tommy delivering a heartfelt message to Allison, a testament to his love and devotion.

Meanwhile, the world outside is revealed to be reeling from the Rapture, with news reports detailing the instantaneous disappearance of children, infants, and Christians, prompting scientists to dub it "Instant Death Syndrome." The atmosphere is further charged by unprecedented weather patterns worldwide. As they struggle to come to terms with this apocalyptic reality, Tommy shares a haunting video of Skylar's abduction by an otherworldly force, with Shay explaining that the Fifth Trumpet has been sounded – and with it, the face of a Fallen Angel has been revealed.

As the congregation huddles together in fear, The Fallen launch a brutal assault on the church, prompting the survivors to flee to the relative safety of the basement. In a selfless act of sacrifice, Shay gives his life to save those he loves.

When morning breaks, Tommy, Jack, Sam, Allison, and Dan set out to find medical aid for Skylar, whose injuries are complicated by a potent demonic venom. Though they reach the hospital, it's too late – Skylar succumbs to her wounds. In a fit of grief and rage, Dan rails against God, only to be impaled and dragged into the sky by an unseen tentacle. Allison falls victim to the same fate.

In a desolate landscape, Tommy, Jack, and Sam stumble upon a relief camp, where Sam shares a final message from Allison: a testament to her unwavering devotion moments before she met her maker. In a poignant act of catharsis, Jack chooses to be baptized, but his newfound faith is short-lived – he's soon claimed by the demonic forces that now roam free.

As "Amazing Grace" echoes through the camp's loudspeakers, demons descend from the sky, drawn to the fervent devotion of believers like Tommy and Sam. In a tragic denouement, the pair makes their final stand against the abyssal horrors, their love for each other and for God ultimately consumed by the very forces they sought to defy.
