Eight years have passed since the MacManus brothers, Conner and Murphy, along with their father Noah, took out the notorious Papa Joe Yakavetta. They’ve been living in obscurity in Ireland, attempting to stay off the radar. However, their peaceful existence is shattered when their uncle Sibeal arrives with troubling news: a respected Boston priest has been murdered, seemingly targeted by an assassin who has adopted the same ritualistic style as The Saints, thereby attempting to frame them for the crime.
Fueled by a sense of duty, the brothers dust off their old gear and weapons and board a container ship back to Boston. During their journey, they encounter Romeo, a Mexican underground fighter who recognizes them and persuades them to let him join the mission. As they listen to news reports, they deduce that Concezio Yakavetta, Papa Joe’s son, is the one behind the murder, intending to draw them out from hiding to enact his revenge.
Meanwhile, in Boston, Detectives Dolly, Duffy, and Greenly investigate the priest’s murder, only to be joined by FBI Special Agent Eunice Bloom, who is eager to prove herself in the shadow of her deceased mentor, Paul Smecker. The team soon realizes that The Saints are not responsible for the killing, launching a hunt to unearth the true assassin. They learn that the assassin is Ottilio Panza, who operates under the command of a shadowy figure known as “The Roman” (played by Peter Fonda).
The brothers and Romeo execute a bold raid on a warehouse being utilized by an Asian gang to manufacture heroin for Yakavetta. After dispatching the gang members, they reconnect with their old bartender friend Doc, who reveals that the priest’s killer is a rogue contractor, not affiliated with Yakavetta. They infer that Yakavetta is hiding in the Prudential Tower, igniting their thirst for justice.
As the action unfolds, The Saints orchestrate a meeting to eliminate Yakavetta’s henchmen, leading to an inevitable showdown where they engage in a life-or-death battle against the underworld’s best. Just when things seem dire, Bloom intervenes to aid them and deduces that the mobsters have turned against each other, framing the narrative in their favor.

In a flashback, Noah opens up about his turbulent history with The Roman—a former ally turned adversary. The plot thickens as Bloom manages to trace Louie’s location.
