Ethan McCoy, portrayed by Nick Nolte, works for the DEA and is conducting a search within a desolate, burned-down house, on the lookout for Cain, played by Josh Duhamel. After failing to find any leads, he instructs his DEA team to locate Cain at all costs. In a dramatic turn of events, we see Cain escaping the house mere moments before it explodes, pursued by a group of armed attackers, resulting in his car being blown up as he flees.

As the scene shifts, we find John Cain waking up in a Mexican hospital. He is disoriented and has no recollection of his identity or the events that led him there. Beside him is a woman named Anna, played by Abbie Cornish, who claims to be his spouse. Alongside them is Eddie, a supposed friend played by Omar Chaparro, who vows to assist Cain in recovering his lost memories. In a bold move, Eddie threatens the attending doctor, demanding that he helps Cain regain his memory, even resorting to blackmail against the doctor’s family.

Anna reveals that Cain was born on December 2, 1972, in Boulder, Colorado. As fragments of Cain’s memory begin to resurface, he starts to sense that those around him are not as they appear. He comes to the unsettling realization that Anna may not be his real wife. Each time he recalls a memory, he notices the doctor administering sedatives, intensifying his suspicions. In a moment of desperation, he confronts the doctor and attempts to flee the hospital with Anna.

However, Eddie and his henchmen soon track Cain down, forcing him into hiding within the hospital’s confines.
