As Ethan McCoy, a no-nonsense DEA agent (Nick Nolte), meticulously searches the charred remnants of a burned-out house, he's on the hunt for Cain - but all he finds are ashes and nothing to incriminate his quarry. Undeterred, McCoy orders his team to track Cain down to the farthest reaches, driven by the knowledge that just minutes prior, Cain had narrowly escaped the inferno, pursued by heavily armed assailants who left his car in smoldering ruin.

Meanwhile, John Cain (Josh Duhamel) awakens in a Mexican hospital, his memories as elusive as the smoke from the burning house. A mysterious woman, Anna (Abbie Cornish), claims to be his wife, but her words are laced with an air of uncertainty. Standing by her side is Eddie (Omar Chaparro), a supposed friend who promises to help Cain regain his lost recollections. As Cain navigates this unfamiliar world, he finds himself under the care of a doctor who seems more interested in preserving his patient's fragile state than aiding his recovery.

Anna whispers secrets to Cain about his past: born on December 2nd, 1972, in Boulder, Colorado - but as fragments of his memory begin to resurface, Cain discovers that those around him are shrouded in deception.
