In a world where fantasy, horror, and steampunk converge, the once-lush village of Malaria has been ravaged by climate change, leaving its farmers destitute. In an attempt to revitalize the economy, the newly appointed King Malbert (Jay Leno) proposes a sinister solution: fostering an industry of malevolent innovations that would sow destruction and enrich evil scientists. This dark vision is further underscored by the dehumanizing treatment of those born with physical imperfections, who are automatically relegated to the status of slave labor, forced to serve under the yoke of their "Igors" – masters notorious for their patronizing condescension.

Yet, amidst this bleak backdrop, an unlikely hero emerges. Igor (John Cusack), once a marginalized outcast, has been secretly nurturing his intellectual curiosity despite being ostracized by his own master, the eccentric Dr. Glickenstein (John Cleese). Behind the scenes, Igor has revived a wisecracking rabbit named Scamper (Steve Buscemi) from its untimely demise, imbuing it with an immortal spirit. He has also reanimated a brain previously belonging to Brian and housed it in a life-support robot, aptly dubbed Brain (Sean Hayes).

As Dr. Glickenstein readies his entry for the annual Evil Scientists Fair – a rocket that would make even the most seasoned innovators jealous – tragedy strikes when a malfunction during a test run claims his life. Recognizing this as his sole opportunity to defy the expectations placed upon him, Igor decides to keep his master's demise under wraps and instead participate in the Fair himself. He constructs a colossal, humanoid entity from human tissue and instills it with the Evil Bone – the key to unbridled malevolence. However, when he brings this creature to life, he discovers that she is anything but evil; instead, she exudes gentleness, politeness, and affection. When Igor attempts to correct her misbehavior by explaining the concept of "evil," she inadvertently transforms it into "Eva" – a name that would become synonymous with her amiable nature.

As Igor hypothesizes that the nefarious Evil Bone had malfunctioned, he promptly whisks Eva away to the "Brain-Wash" to rectify the situation by subjecting her to a barrage of gruesome horror scenes on television. However, Brain's absent-mindedness intervenes, causing him to change the channel mid-treatment, and inadvertently imbues Eva with the persona of a benevolent Hollywood starlet. Seizing this opportunity, Igor deceives her into believing that the Fair is actually an audition for the iconic role of Annie in the renowned musical, and she finally agrees to submit to his commands. As Eva prepares for her role, she forms an unlikely camaraderie with Igor, and finds herself supporting him in his quest for self-transformation.

Seething with frustration, he takes matters into his own hands upon discovering the demise of Dr. Glickenstein and the invention left behind. His initial attempt to seize the creation through force during a high-stakes carriage chase through the rural roads ends in failure. Undeterred, he approaches Igor in person at his laboratory, proposing a deal: hand over Eva as his entry for the Fair, and he will reward Igor with unimaginable wealth. However, Igor uncovers Dr. Schadenfreude's true intentions and narrowly escapes.

As time passes, Igor becomes increasingly introspective, acknowledging that his emotions towards Eva have supplanted his original goals. While standing on his balcony, a visit from Dr. Glickenstein's former assistant, Heidi, serves as a catalyst for his self-reflection.
