In 1993, Dr. Florence Weaver and her colleagues, Victor Fields and John Gregory, worked with a unique psychiatric patient named Gabriel at Simion Research Hospital. Gabriel possessed extraordinary abilities, including controlling electricity and transmitting his thoughts through speakers. However, one fateful night, he unleashed his fury, resulting in the tragic deaths of multiple staff members.

Flash forward twenty-eight years to Seattle, where Madison Lake, a pregnant woman, returns home feeling unwell due to her pregnancy. She resides with her husband, Derek Mitchell. In a moment of rage, Derek violently strikes Madison, causing her to lock herself in the bedroom before drifting off to sleep. Upon waking, she discovers Derek’s lifeless body, just after experiencing a horrifying dream of an intruder committing the act. In a shocking turn, the same intruder attacks Madison, knocking her unconscious.

When Madison regains consciousness in a hospital, her sister Sydney informs her of the tragic news: her unborn child did not survive the brutal assault. Detective Kekoa Shaw and his partner, Regina Moss, interview her. That same evening, Gabriel abducts a woman involved in the Seattle Underground Tour. Disturbingly, Madison dreams again, this time witnessing Gabriel murder Dr. Weaver.

As the investigators look into the case, they uncover a childhood photograph of Madison in Weaver’s home, revealing that Weaver had specialized in child reconstructive surgery. The sisters decide to approach the police again after witnessing Gabriel kill Fields. Gabriel reaches out to Madison, triggering buried memories of her childhood. To piece together her past, Madison and Sydney seek out their mother, Jeanne, where Madison comes to the startling realization that Gabriel, initially thought to be an imaginary friend, was indeed real and part of her life.

Shaw and his team connect the dots linking Madison with the deceased doctors, leading them to uncover Gregory’s body. They engage a psychiatric hypnotherapist in hopes of unlocking Madison’s repressed memories. During this process, Madison discovers that her birth name is Emily May, and in a shocking revelation, learns that Gabriel had intended for her to kill her unborn sister. Just as she may have given in, Madison managed to resist.

The situation escalates when the kidnapped woman unexpectedly falls from the attic, revealing that Gabriel had been living in Madison’s home. It is further disclosed that the woman is Serena May, Madison’s biological mother. Sydney investigates the now-closed Simion hospital and learns that Gabriel is Madison’s twin brother, living as a parasitic entity inside her, following a procedure Dr. Weaver performed to remove him from her body. He lay dormant during her childhood but awakened due to a traumatic incident with Derek.

After provocation by inmates at the facility, Gabriel seizes control of Madison’s body, using his incredible powers to massacre everyone in the precinct. As he escapes with stolen clothes and weapons, Sydney and Shaw attempt to stop him at the hospital where Serena is being treated. In a fierce confrontation, Shaw sustains serious injuries, and Sydney is trapped against the wall by a thrown hospital bed.

In a twist of fate, Sydney reveals to Madison that Gabriel was responsible for her miscarriages, feeding off her unborn babies. This revelation ignites Madison’s fury, allowing her to reclaim authority over her body. Together, they enter a surreal mental landscape where Madison imprisons Gabriel, asserting her dominance over him and seizing his powers for herself.

Leaving Gabriel trapped in this mental realm, he warns her that he will return. Madison confidently retorts that she will be prepared when he does. Once back in the hospital, Madison utilizes her newfound superhuman strength to lift the bed pinning Sydney, and the sisters embrace, with Madison affirming their bond. Despite her adoption and the lack of blood ties, she declares her pride in their relationship.
