As John tracks down Tom to his home, the two engage in an intense and brutal battle. Despite being shot multiple times and pushed down a flight of stairs, Tom ultimately falls victim to John’s wrath. Tom’s wife Nancy stumbles upon the gruesome scene and is also brutally killed by John. Just as Alison arrives at her brother’s house to pick up her belongings for their trip, she discovers the lifeless bodies of her loved ones and flees in terror as Dan and Charlie arrive on the scene with the police.

As chaos ensues, Dan and Charlie take Alison to the institution, unaware that John has also returned there, driven by a thirst for revenge against those who have wronged him. In a desperate bid to restore order, Phillip leaves to examine samples while Paul attempts to eliminate John by injecting him with acid. However, John proves resistant to this attack and kills Paul after a brief struggle by stabbing him with the syringe. Following this gruesome event, Phillip returns to his office, where he briefly converses with John about the success of their experiment. Initially, John seems to grasp the significance of their work together, but ultimately snaps Phillip’s neck in a brutal display of rage.

As Dan investigates at the county coroner’s office, Charlie and Alison stumble upon John brutally killing another institution worker; Charlie valiantly attempts to apprehend him but is mortally wounded when John breaks his back.
