As the Vietnam War’s tumultuous final stages come to a close, a weary American military squad receives orders to secure a ravaged village from the impending North Vietnamese forces. In the midst of this chaos, Private Luc Deveraux (Jean-Claude Van Damme) stumbles upon a gruesome scene: his comrades and innocent villagers lie lifeless, their ears brutally severed. The culprit behind this heinous act is revealed to be none other than Sergeant Andrew Scott (Dolph Lundgren), who has succumbed to the horrors of war and descended into madness. Deveraux finds Scott holding a string of mangled ears aloft, accompanied by the chilling sight of a young boy and girl held hostage.

A desperate attempt at reason ensues as Deveraux implores Scott to recognize that the war is over, but his words fall on deaf ears. In a bizarre twist, these captives are forced to witness the cold-blooded murder of one of their own as the terrorists continue to demand concessions.

In response to this crisis, authorities order law enforcement agencies to stand down and refrain from attempting any rescue operations. Instead, they call upon the Universal Soldiers (or “UniSols”), an elite counter-terrorism unit comprised of operatives like Deveraux (now designated GR44) and Scott (now designated GR13). The peculiarity of this situation lies in the fact that these UniSols have been revived with no recollection of their past lives. Deveraux, however, begins to experience fragments of his forgotten existence, including a deleted scene where he encounters a Vietnamese bar girl named Linh at Lucky’s, a seedy establishment notorious for its shady dealings and illicit pleasures.

The sudden stirrings of memory cause him to turn away from Scott, who is equally haunted by his own experiences. This unsettling silence, however, is met with radio silence from their handlers back at the command center.

Meanwhile, behind the scenes, the UniSols’ technicians are grappling with the mystery of the Hoover Dam incident. They reveal that these advanced soldiers possess extraordinary regenerative abilities and superhuman strength, but also a propensity for overheating and shutdowns. In a separate thread, Veronica Roberts (Ally Walker), a tenacious TV journalist, is desperate to revive her career by uncovering the truth about the UniSol project. Her initial attempts are rebuffed by evasive officials, prompting her to take matters into her own hands and sneak onto the base to gather evidence. There, she stumbles upon a severely damaged UniSol whose very existence seems to defy logic – it’s as if this mechanical warrior is refusing to surrender, its life force ebbing away in slow motion.

As the desperate duo - Deveraux and Scott (Michael J. Fox) - attempts to make their escape, they’re met with a stern ultimatum: bring back the elusive Roberts. As Deveraux and Roberts continue their quest to uncover the truth about the UniSol program, they cross paths with Dr. Gregor (Jerry Orbach), the mastermind behind the operation. He regales them with tales of the project’s origins, revealing secrets that threaten to upend everything Deveraux thought he knew about his own past.

Emboldened by this newfound understanding, Deveraux sets out to rejoin his family in Louisiana, only to have Scott track them down and take hostages. A brutal showdown ensues, with Scott mercilessly pummeling Deveraux due to the muscle-enhancing effects of the UniSol serum. Roberts manages to escape her bonds, but not before being seemingly obliterated by a well-placed grenade courtesy of Scott.

In a stunning turn of events, Deveraux seizes control of his own fate, injecting himself with a UniSol serum and summoning the strength to take down Scott in a fierce battle. As the dust settles, Deveraux delivers the final blow, crushing Scott beneath the merciless grind of a meat grinder. With her captor vanquished, Roberts emerges from the wreckage, wounded but alive. The Special Edition DVD release of this gripping thriller presents an alternative conclusion that unfolds in the aftermath of Scott’s brutal hostage-taking escapade. As Deveraux seizes control of the situation by grabbing a shotgun in the kitchen, the front door swings open to reveal his mother, only for Scott’s ruthless instincts to take her life with a fatal shot.
