Many years following the Rapture, the surviving remnants of humanity face a grim fate as they contend with the Burned Ones—demonic, humanoid beings that thirst for human flesh and blood. The story follows Azrael, portrayed by Samara Weaving, and her partner Kenan, played by Nathan Stewart-Jarrett, who are uprooted from their forest-dwelling cult. This extremist group believes that speech itself is a sin, going so far as to surgically remove their vocal cords to maintain silence. In a power play, Josephine, one of the cult’s rulers, captures the couple with the aid of her enforcers and plots to sacrifice Azrael to the lurking Burned Ones. Undeterred, Azrael fights back and escapes, killing one of Josephine’s henchmen in the process.
As she flees through the dense woods, Azrael manages to infiltrate the cult’s fortified encampment, where she encounters Miriam, the cult’s spiritual interpreter. Miriam claims to receive divine guidance through the whispers of the wind as she listens intently through a hole in the wall of a dilapidated church. Just as Azrael forms a plan to confront the cult, Josephine catches her, forcing Azrael to make an escape under dire circumstances. In a fateful twist, when she uncovers a road, a kind stranger offers her a ride, only for tragedy to strike as cultist gunfire leads to a horrific crash, leaving Azrael to fend for herself once again.
In the heart of the woods, Azrael is devastated to discover Kenan nailed to a tree, but her attempts to save him are thwarted when she falls into a trap set by the cultists. Suspense mounts as the Burned Ones arrive, tearing apart the cultist, but tragically killing Kenan in the process. In a desperate fight for survival, Azrael manages to elevate herself to safety by climbing a tree, killing one of the Burned Ones along the way.
Determined, Azrael returns to the cult encampment and finds herself face-to-face with Miriam in the church. Intent on vengeance, she nearly executes Miriam but hesitates upon noticing her pregnancy. This moment of mercy allows Miriam to turn the tide, incapacitating Azrael, who in a moment of desperation scratches Miriam’s pregnant belly. Under Josephine’s command, the cultists entomb Azrael in a coffin attached to a tunnel, where a Burned One approaches her but abruptly recoils from the smell of Miriam’s blood on Azrael’s hand.
In a fury, Azrael manages to wriggle free from her confined space, returning to the encampment with a blazing inferno, systematically taking out the cult members. The tension escalates as she once again confronts Miriam, leading to a savage clash where Azrael viciously bites into Miriam’s neck, causing grievous harm. As the chaos unfolds, Miriam goes into labor while an increasingly injured Josephine arrives to assist her, leading to a dramatic confrontation. In a climactic moment, Azrael fatally stabs Josephine in the neck, just as the cries of Miriam’s newborn echo through the church. In a shocking turn of events, Miriam, horrified by her own child, takes her life.
As Azrael approaches the newborn, she uncovers its true, horrific nature—a creature resembling the Antichrist. The atmosphere thickens with dread as the Burned Ones pour into the church, their wails rising as Azrael stands defiant, cradling this monstrous infant.
