A young boy named Thomas is taken in by Ava Paige and her WCKD organization troops, along with other children, after his mother leaves him for his own safety. Years later, shortly after the remaining Gladers escape from The Maze, the now teenage Thomas (Dylan O’Brien) and Teresa Agnes (Kaya Scodelario) — former employees of WCKD and architects of the Maze — reunite with friends Newt (Thomas Brodie-Sangster), Minho (Ki Hong Lee), Frypan (Dexter Darden), and Winston (Alexander Flores) at a facility managed by Mr. Janson (Aidan Gillen).
Mr. Janson reassures them that the facility serves as a refuge for those immune to the Flare virus, providing safety from WCKD and the infected called Cranks while supplying food, clothing, and sanitation. Curious about the true nature of this haven, Thomas stumbles upon the reality hidden behind these walls with the help of Aris Jones (Jacob Lofland), the first Maze escapee to arrive at the facility. They uncover shocking news: Ava is still alive, and Mr. Janson is carrying out cruel experiments on the Immunes under WCKD’s orders, hunting for the elusive resistance group known as The Right Arm located in the mountains. Ava instructs Janson to start harvesting the Immunes, beginning with the latest arrivals, including Thomas and his friends.
In a desperate bid for freedom, the group, along with Aris, escapes with Janson desperately sending WCKD troops after them, leading them into the barren wasteland known as the Scorch. They come across a desolate marketplace, where they first confront the horrifying Cranks, individuals grotesquely transformed by the Flare. In a tragic turn of events, Winston becomes infected during this encounter, compelling the group to leave him behind after he chooses to end his life rather than transform into a monster.
As they press deeper into the wasteland, Thomas is jolted awake by the sight of civilization shimmering on the horizon one night. With a looming thunderstorm, they rush to seek refuge in an abandoned facility. There, they are relieved to find Minho surviving a near-fatal lightning strike, only to discover the place is swarming with chained Cranks.
In a surprising turn, they meet Brenda (Rosa Salazar) and Jorge (Giancarlo Esposito), who lead a perilous group of survivors and have effectively utilized the Cranks as their guards. Upon learning about the group’s journey from WCKD seeking The Right Arm, they agree to guide them, abandoning their own crew. Just then, Janson and WCKD storm the facility, but Jorge’s pre-placed explosives create chaos, allowing most to escape. However, Thomas and Brenda are left behind, and in the fray, Brenda gets infected.
Haunted by memories of working with Teresa during their time at WCKD, Thomas recalls trying to alert her of the organization’s sinister intentions just before they were separated. Upon reuniting with the others, Jorge confronts Marcus (Alan Tudyk), who is revealed to be a WCKD spy. Eventually, they reach the Right Arm’s refuge, where they meet Vince (Barry Pepper), in charge of their fates. As Brenda’s condition worsens, Vince contemplates shooting her, but is interrupted by Mary Cooper (Lili Taylor), a former WCKD scientist who identifies Thomas as their informant.
Mary successfully treats Brenda using a special enzyme that can only be found in the bodies of Immunes. She explains that her departure from WCKD resulted from disagreements with Ava about how to produce a cure. Meanwhile, Teresa, believing in the goodness of WCKD’s mission, betrays their whereabouts, leading to an ambush by WCKD forces. Ava and Janson arrive to capture survivors for experimentation, and in the ensuing chaos, Janson fatally shoots Mary. After a fierce fight, Thomas and the Right Arm manage to repel the WCKD attackers, but Ava, Janson, and Teresa retreat, taking captives with them.
Now, left with only a handful of survivors, Thomas hatches a plan to infiltrate WCKD, determined to rescue his friends and reclaim civilization, with the backing of Vince and the Right Arm, they gear up for the inevitable battle ahead.
