Will, a bartender in New Orleans, finds himself immersed in chaos while working at a deteriorating bar plagued by cockroaches. One fateful night, the bar sees a mix of patrons, including Alicia, her boyfriend Jeffrey, and a drunken man named Eric, who soon gets into a violent altercation, resulting in him being stabbed in the cheek with a shattered bottle. Panic ensues as college students flee the bar, leaving behind a forgotten phone.
Taking a moment to inspect their abandoned device, Will encounters unsettling messages from a user named Garrett, who expresses fear and claims to be pursued by something dark from the “tunnel.” The next morning, Will wakes to a chilling photo sent by Garrett—a gruesome pile of bloody teeth. Alarmed, Carrie, Will’s girlfriend and a student, insists on contacting the police immediately. After leaving Carrie at school, where he suspects her professor is involved with her, Will visits Eric, who is suffering from an infected wound and tormented by nightmares. A shocking discovery is made when Will observes something otherworldly emerging from Eric’s injury.
Throughout the day, tension builds as Will becomes increasingly aware of an unsettling stalker among the college students. Investigating the phone reveals disturbing imagery of a corpse and a decapitated head amidst cockroach swarms. Despite Carrie’s insistence on reporting this evidence to authorities, the situation escalates when they encounter a book titled “The Translation of Wounds” linked to the sinister happenings. Desperate, they try reaching out to Garrett, only to be met with a screeching noise on the line.
While en route to the police station, Will realizes he’s being followed yet again, receiving strange messages labeling him “chosen.” In a moment of hysteria, he hallucinates the phone transforming into an eerie creature—he discards it, but the action proves futile as the police are left powerless without it.
With emotional ties strained, Will and Alicia briefly engage in a tender moment which quickly turns sour, overwhelming Alicia with guilt. Will discovers Carrie in a bewildered state at their home, entranced by a disturbing video of a tunnel. This uncharacteristic behavior ignites concern as she cannot recall sending him cryptic messages while in a trance.
As the night progresses, Will experiences a frightful encounter with Garrett, who divulges dark secrets of possession and ritualistic summoning, claiming that Will has unwittingly invited malevolent forces into his life. In a terrifying revelation, Will’s own body becomes a canvas for these horrors, as he hallucinates a cockroach crawling out of an armpit wound.
As their relationship deteriorates, tensions rise between Will and Carrie. Struggling to cope, he witnesses her obsession with Garrett, alongside research into arcane texts alluding to ritualistic wounds being gateways for other entities.
At work, Will’s mounting aggression culminates in a violent outburst that prompts him to quit his job. Meanwhile, Alicia urges him to cease his calls, pushing him further into isolation. Seeking refuge at Eric’s increasingly troubling home, Will uncovers that Eric was manipulated by the college students in a sinister plan to draw Will into a horrifying trap.
As Eric’s condition worsens and the presence of the higher being reveals itself through Eric’s wound, Will grasps the severity of the threat looming over them all. After another chilling call to Garrett prompts an onslaught of cockroaches to invade the room, Will acknowledges the eldritch force he is battling—he finally surrenders to his fate, aiming to absorb the entity in a desperate quest for wholeness, as darkness consumes the house around them.
