In 1962, in a quaint rural town, the local teenage boys take part in the annual Halloween event known as the “Run.” This competition demands that they stop a deadly supernatural entity called Sawtooth Jack from reaching the church before the clock strikes midnight. This year, Jim Shepard emerges as a hero after slaying the creature, leading the boys to feast on its candy-filled insides, thus securing an impressive reward of $25,000 and a brand new Chevrolet Corvette from the Harvesters Guild. As the town indulges in the festivities, Jim bids farewell to his younger brother, Richie Shepard, and drives off into a promising future.
Fast forward a year later, and Officer Ricks (Luke Kirby) addresses the local boys about the Run. However, Richie is exempt from the talk since his family has already claimed victory. Amidst this, Richie finds himself enchanted by Kelly, a new arrival in town, and feels an overwhelming urge to participate in the Run, despite his parents’ warnings against it. After enduring a beating from his classmate Riley (Austin Autry), who steals his belt, Richie becomes plagued by postcards and a letter from Jim. In a moment of desperation, he attempts to flee in a stolen vehicle, only to be apprehended by Ricks.
Meanwhile, a mysterious farmer resurrects an emaciated corpse, transforming it into a scarecrow with a carved pumpkin head and stuffing it with candy before it arises as Sawtooth Jack. True to tradition, the town’s boys are locked away and deprived of food for three days until Halloween night, when they are unleashed onto the streets wearing masks and wielding improvised weapons. In a bid to join his friends Mitch, Bud, and Charlie, Richie sneaks out, only to encounter Sawtooth Jack in a cornfield. Tragically, Charlie and Mitch meet their end at the hands of the creature, with Richie appearing to escape unscathed.
Richie soon reunites with Kelly, who also seeks to join the hunt, while a traumatized Bud tries to return home, only to be denied entry by his parents. In a desperate plea, he seeks refuge in a storm cellar, but Sawtooth Jack soon finds him, leading to the gruesome deaths of Bud and his friends. Amid the chaos, Richie and Kelly steal a handgun from Ricks’ house and commandeer his patrol car as the increasingly wild boys begin looting and even turning on one another. After confronting Riley, Kelly knocks him out, allowing Richie to reclaim his stolen belt buckle.
Ricks, using his car’s radio to lead Richie and Kelly to the town center, reveals the presence of Sawtooth Jack. As Richie prepares to shoot the creature, he gets a shocking revelation: this year’s Sawtooth Jack is none other than his brother, Jim. As chaos unfolds, Richie and Kelly confront his mother, Donna (Elizabeth Reaser), only to learn she was behind the postcards and letters from Jim. Overwhelmed by remorse, Donna inflicts harm upon herself, while Ricks warns Richie’s father, Dan (Jeremy Davies), of the urgency to complete the Run to save the town.
As Richie and Kelly make their way to the church, Dan unveils the disturbing truth behind the town’s curse: Ricks and the Harvesters Guild have tied the community to this gruesome ritual, ensuring Sawtooth Jack is always vanquished while the latest victor becomes next year’s monster. Determined, Richie lures Sawtooth Jack to the church to disrupt the cycle, but Ricks and Riley intervene, and Riley shoots Sawtooth Jack. In a moment of finality, Richie aims his gun at Riley, only to be beckoned by Sawtooth Jack for mercy; ultimately, Richie delivers his brother a fatal shot to the head.
As the town rejoices, Richie and Kelly secretly plan to escape together, where she slips him a gun for protection. Presented with his earnings, Richie drives away in his new car to find Kelly, but Ricks intercepts him while she hides away. In a fight for survival, Richie fires first, taking down Ricks, but is subsequently ambushed by the farmer and buried alive. Urged by Dan, the heartbroken Kelly speeds away in Richie’s car. The next year, as Richie’s body transforms into Sawtooth Jack, Dan takes matters into his own hands, slaying the farmer and pushing his son to exact revenge on the town that betrayed him.
