On a chilling winter night in 1953, three children—Anna, her brother Max, and their friend Mary—are engaged in a sinister game in Anna’s attic. They form a protective circle of salt as they struggle to light candles to ward off the Midnight Man, a malevolent entity that ultimately claims Mary’s life and pursues the other two. Unfortunately, Max’s decision to leave the attic through the main door breaches the game’s rules, sealing his fate as well.
Fast forward to the present day, another cold winter night finds a now senile Anna in bed, attended to by her teenage granddaughter, Alexandra ‘Alex’. During a phone call, Alex’s friend Miles expresses his desire to visit. However, upon hearing strange noises, Alex rushes to Anna, who requests her hand mirror from an old chest in the attic. Inside, Alex discovers remnants from the past, including a shotgun, bullets, and a mysterious, tightly sealed box. When Miles joins her, they pry it open to reveal a disturbing assortment of items: old photographs, candles, matches, a salt shaker, and a torn sheet detailing the rules of a game known as The Midnight Man. To their horror, there are also names inscribed alongside drops of blood. Anna is horrified upon interrupting them and immediately faints.
Summoned by Alex, family friend Dr. Harding and his assistant Alan arrive to check on Anna’s condition. Dr. Harding extends his condolences to Alex for the loss of her mother during her childhood. Intrigued by the game’s secrets, Alex and Miles decide to follow the instructions they stumbled upon, entering their own names and blood into the list before knocking on the front door at the stroke of midnight, all while Anna listens helplessly from her room. The candlelit atmosphere leads to an eerie silence until Anna unexpectedly speaks through the interphone. When Alex returns to her, she finds the room deserted.
In a frantic search, Alex heads to the bathroom, where she is suddenly ambushed by the Midnight Man, who drenches her in blood as Anna blows out her candle. Meanwhile, Miles discovers the darker aspects of the game’s rules in the attic. In an impulsive moment, he extinguishes his candle to test the game’s credibility and faces torment from the supernatural foe. In a panic, Miles escapes and reunites with Alex; together they grasp the terrifying reality that the game they’re playing is all too real.
Soon after, their friend Kelly arrives, having been invited by Miles earlier. Despite her skepticism and their warnings, she reluctantly joins the unfolding nightmare. The three venture into a room plagued by pipe leaks, where their candles flicker ominously as the Midnight Man draws near. Alex is the only one to see him when she gazes into the hand mirror. Just as panic ensues, a leak extinguishes Kelly’s candle. They frantically form a circle of salt around Kelly for protection and head out to find another candle. However, the Midnight Man ultimately disrupts their efforts, shattering the circle and unleashing horrific visions from Kelly’s past.
Desperate for answers, Alex and Miles encounter Dr. Harding again in the sitting room. He reveals that he was there the night Anna first played the game but had been denied participation. He explains that Anna’s repeated attempts to play over the years led to tragic fates for many, including Alex’s mother, who had taken her own life due to the Midnight Man’s relentless grip. As their urgency grows, the trio discovers Kelly’s lifeless body, her throat slit. Dr. Harding insists they must destroy the game, but Anna suddenly appears in the attic, fatally attacking Harding before Alex shoots her with the old gun.
With darkness looming, the Midnight Man emerges, taunting them. The clock strikes 3:30, just moments before the game concludes, prompting him to vanish. They wait anxiously until 4:00, ready to escape the circle. Yet, when Miles notices that Harding’s watch reads 3:27, he realizes they’ve been outsmarted. In a cruel twist, the Midnight Man claims Miles and confronts Alex, declaring the game over.
Later, we see Alan and his son at a garage sale outside the house. Alan recounts stories about the previous residents to his son, who stumbles upon the ominous box, still tightly closed. Shaking it in innocent curiosity, he asks if he can take it, to which Alan consentingly agrees, oblivious to the horrors contained within it, leaving us with a chilling echo of hope that it’s merely a game.
