In the unforgiving landscape of Mars, amidst the Union Aerospace Corporation’s Olduvai Research Facility in 2046, a sense of unease settles over the scientists as they are brutally dragged into the shadows by an unseen terror. Dr. Carmack (Robert Russell) frantically transmits a warning about a catastrophic Level 5 security breach mere moments before the door behind him is torn asunder, revealing a horror beyond comprehension. His gaze freezes in terror as he beholds something unfathomable through the jagged opening.

Meanwhile, on Earth, a team of eight elite Marines, known as the Rapid Response Tactical Squad (RRTS), are poised to embark on leave, their anticipation palpable. However, their respite is abruptly terminated when their squad leader, Sarge (Dwayne Johnson), receives devastating news: contact with Olduvai has been lost, and the RRTS is hastily dispatched to investigate the mysterious disappearance. As a result of the crisis, the Mars station is effectively quarantined, isolating its 85 UAC employees from the rest of humanity.

As the Marines prepare for their perilous journey, Sarge takes John “Reaper” Grimm (Karl Urban) aside and implores him to reconsider his participation in the mission, citing Reaper’s sister, Dr. Samantha Grimm (Rosamund Pike), who is stationed on Mars. Despite Sarge’s heartfelt plea, Reaper remains resolute in his determination, ultimately joining the team as they suit up for their ill-fated expedition.

The Marines’ fate is sealed when they are teleported to Mars via the enigmatic “Ark” device, located at Area 51, a mysterious technology discovered in 2026. During their journey, they encounter Pinky (Dexter Fletcher), an early victim of the Ark’s unpredictable teleportation events, whose lower torso has been transformed into a peculiar two-wheeled engine, reminiscent of a Segway. Upon arrival on Mars, they are met with the eerie silence and foreboding atmosphere that shrouds the abandoned research facility.

As the team embarks on their perilous mission to eliminate the threat, secure the facility, and recover UAC property, Reaper engages in a poignant conversation with his sister, revealing a shocking discovery: humanoid remains unearthed on Mars, bearing a 24th chromosome that imbues creatures with superhuman attributes. This extraordinary finding sparks a sense of unease, as it becomes clear that these beings are impervious to disease and can rapidly recover from injuries.

Meanwhile, the team locates Carmack, whose mental state has devolved to the point of self-mutilation - he’s torn his own ear off in a fit of madness. The Marines return him to the lab, where Grimm attempts to sedate him, but not before they discover that the creatures encountered are not mindless beasts, but rather intelligent entities driven by a singular purpose: to eliminate them.

As the team delves deeper into the facility, they uncover the dark truth behind the Martian chromosome’s influence. Blood samples taken from two hostile creatures reveal that they were once human, their genetics altered by the addition of the 24th chromosome - a process seemingly determined by the chromosome itself, capable of distinguishing between those with the capacity to be evil or not.

The Marines soon realize that Carmack had deliberately injected the chromosome into an “evil” subject (Curtis Stahl, a multiple murderer), who later transformed and broke free. This disturbing revelation sets the stage for a series of gruesome events, as the team faces off against hordes of Imps, determined to kill them.

The stakes are raised when Mac, Destroyer, and Portman fall victim to the creatures’ relentless attacks, leaving only Sarge, Reaper, The Kid (Al Weaver), Duke (Raz Adoti), and Sa to carry on. As the surviving members regroup, they discover that a Hell Knight has escaped through the Ark, making its way back to Earth due to Pinky’s refusal to trigger Mac’s grenade, which would have destroyed the Ark.

In response to this dire situation, Sarge seizes control of the Bio Force Gun, dubbing it the “Big Fucking Gun.” As they navigate the treacherous landscape, the team stumbles upon a UAC facility on Earth overrun by corpses. In a desperate bid to prevent the infection from spreading, Sarge orders his men to kill anything still alive.

The Kid’s discovery of non-infected humans sparks hope, but his refusal to slaughter them earns him a deadly reprisal at the hands of Sarge. Pinky reappears, aiming a pistol at Sarge, only to be dragged off by a monster as the team descends into chaos.

As zombies close in, Duke meets a gruesome end, and Sarge is dragged through a malfunctioning bulletproof screen. Reaper takes a ricochet hit, leaving him on the brink of death. In a last-ditch effort to save his life, Sam injects Reaper with the Martian chromosome, but instead of unleashing a monster, Reaper awakens as a superhuman, imbued with enhanced strength and healing abilities - a transformation that may hold the key to their survival.

As Reaper’s newfound strength takes hold, the narrative shifts to a visceral, action-packed perspective, plunging viewers into the midst of a merciless killing spree. With ruthless efficiency, Reaper dispatches a plethora of monstrous adversaries, including Pinky, who has grotesquely mutated into a demonic entity, its lower half dragged behind it like a gruesome appendage. The relentless assault culminates in a slaughterhouse scene, replete with mangled corpses and the eerie aftermath of a BFG blast, leaving a trail of destruction in its wake.

As the camera returns to a standard perspective, Reaper emerges from the facility’s exit, surveying the carnage that surrounds him. The bodies strewn about are a grim reminder of the devastation wrought by the relentless onslaught, while the blue hole in the wall serves as a testament to the destructive power of the BFG blast. Upon encountering Sarge and an unconscious Grimm, Reaper demands to know the fate of the non-infected survivors, only to be met with a chilling revelation: Sarge had taken their lives.

As Reaper’s gaze falls upon Sarge’s injuries, he is struck by a familiar sight - the same telltale signs that Carmack bore before his transformation into an Imp. It becomes clear that Sarge has been hiding this truth, even as Grimm, badly injured and struggling to move, crawls towards safety. The Marines’ uneasy standoff ends with Reaper pinning Sarge against the wall, whereupon the latter’s metamorphosis begins in earnest. As Reaper watches, Sarge’s features contort into a grotesque parody of his former self: a prominent skull structure, sharp teeth, and glowing red eyes all testify to his impending transformation.

Reaper seizes the opportunity, hurling Sarge into the Ark before following up with a well-placed grenade, reducing both the traitor and the Ark to smoldering ruin. With Sarge eliminated, Reaper tenderly retrieves Sam, who though conscious is unable to walk due to her injuries. As he cradles her in his arms, Reaper begins his ascent back to the Earth’s surface, a somber figure haunted by the carnage that surrounds him and the secrets he has uncovered.
