Kate Beckinsale plays Lindy Lewis, a young woman plagued by intermittent explosive disorder, a condition that renders her prone to violent outbursts due to an unusually high level of Cortisol coursing through her veins. This physiological anomaly grants her extraordinary physical prowess, but also makes her a ticking time bomb, waiting to unleash fury at the slightest provocation. As her erratic behavior becomes increasingly unmanageable, Lindy’s parents are forced to institutionalize her, and even the army is unable to tame her wrath.

In an effort to curb her destructive tendencies, Lindy undergoes an innovative treatment - a form of self-administered electroconvulsive therapy delivered through a specially designed vest. This innovative approach enables her to lead a relatively normal life, free from the constraints of her unpredictable rage.

Some time later, Dr. Ivan Munchin (Stanley Tucci), Lindy’s therapist and the creator of her treatment, prompts her to embark on a date with accountant Justin (Jai Courtney). Munchin believes this will serve as a litmus test for Lindy’s ability to form a healthy, normal connection with another individual. After an uneventful initial encounter, which is disrupted by their waitress’s condescending attitude towards Justin’s nut allergies, Lindy and Justin agree to meet again.

Upon returning to Dr. Munchin, Lindy discovers that her tolerance to the electroshock therapy has increased, prompting him to raise the dosage. Munchin encourages Lindy to pursue a relationship with Justin, citing their shared past experiences (Lindy’s background as a bouncer) and Justin’s unique profession, which revolves around a single client, Barry.

As Lindy and Justin grow closer, they find themselves back at her apartment, where their intimacy is interrupted by the presence of Lindy’s voltage therapy vest. Despite this peculiarity, Justin downplays its significance, initiating a sequence of events that will challenge the boundaries of Lindy’s newfound sense of normalcy.

Desperate for answers, Lindy turns to Dr. Munchin once more, claiming that the vest’s efficacy has waned. The perceptive doctor, sensing her turmoil, prescribes a sedative to help calm her troubled mind. When she awakens, Lindy is consumed by an insatiable desire to uncover the truth behind Justin’s murder. Despite Dr. Munchin’s warnings that pursuing justice may irreparably alter her, Lindy becomes fixated on finding the killer.

Under the guise of offering additional insights into the case, Lindy contacts Detective Vicars and arranges a meeting at the precinct. Her true intention is to gain access to the evidence room, where she hopes to uncover crucial information about Justin’s murder. Following a brief, high-stakes encounter with Detective Nevins (Laverne Cox), Lindy manages to evade capture.

Seizing an opportunity, Lindy commandeers Justin’s car and embarks on a thrilling getaway, narrowly avoiding a confrontation with Detective Vicars after he sustains injuries during the chase. As she flees, Lindy finds herself at an electronics store, where she encounters Andi, a precocious young hacker (a little girl) who is instrumental in helping her uncover the truth.

Through their collaboration, Lindy discovers that Justin’s last known contact was none other than Barry Kasparzki, a notorious gangster whose reputation precedes him. As the stakes continue to escalate, Lindy finds herself hurtling toward a confrontation with the dark forces that have claimed Justin’s life.

Her next move takes her to Detective Vicars’ hospital bedside, where she learns that Fizel is hiding in plain sight. The detective’s willingness to aid her stems from his conviction that Lindy genuinely cared for Justin.

However, their conversation is interrupted by the sudden arrival of Detective Nevins, who pursues Lindy with reckless abandon. In a desperate attempt to stall him, Lindy resorts to an unconventional tactic: she begins hurling infants at Nevins in the maternity ward, buying herself precious time.

Lindy’s quest for justice takes her to Fizel’s stronghold, where his head of security, Delacroix (Ori Pfeffer), awaits. He renders her unconscious and transports her back to her apartment, absconding with her electroshock vest in the process. Meanwhile, at Fizel’s headquarters, the enigmatic mogul instructs Delacroix to eliminate Lindy once and for all.

Back in the car, Lindy’s driver refuses to heed her demands to be released, prompting her to take matters into her own hands by choking him until he loses control of the vehicle. As a result, the car crashes, giving Lindy the opportunity to slip away.

In search of a new electroshock vest and a chance to regroup, Lindy visits Dr. Munchin before returning to her apartment. The discovery of the 9 sign on her door, now relocated from its original position, suggests that someone has been monitoring her activities. Undaunted, Lindy infiltrates her apartment through the window and discovers that Fizel’s operatives have rigged an explosive device at the entrance.

With her quick thinking and resourcefulness, she disarms the deadly trap and pockets it for future use. Her subsequent departure is marked by a chance encounter with Detective Vicars outside her apartment building. He cautions her against seeking to evade arrest, citing his willingness to go easy on her due to her unique circumstances and his personal distaste for Nevins’ vitriolic assessment of Lindy’s character.

Just as she appears poised to make her next move, Lindy’s apartment blows up in a spectacular explosion, prompting her to flee the scene once again.

The cold, calculating villain takes pleasure in explaining the gruesome concept of Exsanguination to her, as if it were a morbid academic exercise. But Lindy refuses to be cowed, breaking free from his grasp and launching a fierce counterattack against her tormentor and his guards.

As she fights her way out of Fizel’s lair, Lindy discovers that Justin’s apparent demise was merely a ruse, engineered to allow him to strike at the heart of their nemesis. The revelation sparks a fire of determination within her, driving her to seek vengeance against those who have wronged her. Her pursuit leads her to Dr. Munchin, whose sinister connection to Fizel and the CIA is finally exposed.

Justin’s betrayal cuts deep, but Lindy’s rage is tempered by a newfound understanding: she was merely a pawn in a larger game, a test subject for the cruel experiments conducted by Dr. Munchin.
