Dr. Benjamin Stone, portrayed by Michael J. Fox, is a talented young surgeon who’s tired of the monotonous emergency room life in Washington, D.C. Eager for a change, he dreams of becoming a plastic surgeon in Beverly Hills to pay off his med school debts while enjoying a more glamorous lifestyle. However, on his final day at the hospital, he senses a lack of warmth from his colleagues. To add insult to injury, a farewell cake is humorously defaced with “Good riddance, asshole.”

His grand plans take a detour when a cross-country drive in his classic 1956 Porsche turns disastrous, landing him in the quirky small town of Grady, South Carolina, after crashing into the fence of local Judge Evans (Roberts Blossom). His humble offer to repair the fence backfires as the irate judge assigns him 16 hours of community service at a nearby hospital, which escalates to 32 hours due to his growing frustration. As he arrives at the hospital, the uncompromising Nurse Packer (Eyde Byrde) swiftly puts him in his place, treating him like a regular worker with a strict clock-in and clock-out regime.

Despite his initial resistance, Ben forms a bond with the town’s charming Mayor, Nick Nicholson, played by David Ogden Stiers, who sees potential in Ben to fill the shoes of the aging local doctor, Aurelius Hogue, portrayed by Barnard Hughes. During a warm welcome at the mayor’s lakeside lodge, featuring homemade hospitality from café owner Frances Sternhagen, Ben meets the spirited ambulance driver, Vialula “Lou,” played by Julie Warner. After a night of drinking, Ben wakes up to find Lou emerging from the lake, marking a significant moment in his journey.

As Ben begins treating patients in the relaxing town atmosphere, he encounters simple medical cases that reveal the town’s unique character—a far cry from the intense, high-stakes environment he is used to. He faces a humbling moment when he mistakenly misdiagnoses a young boy, ultimately leading to an embarrassing realization about the boy’s condition being a result of swallowed tobacco after consuming soda, which turns out to be the unwittingly effective treatment suggested by Hogue.

Amidst his community service, Ben becomes entangled with the mayor’s flirtatious daughter, Nancy Lee, played by Bridget Fonda, who quickly takes an interest in him. However, his heart is set on Lou, even as he discovers the complications of her past, including her role as a single mother. The rivalry intensifies with local “golden boy” Hank Gordon, portrayed by Woody Harrelson, who also has feelings for Lou.

Their bond deepens, and Ben confesses the tragic loss of his parents in a car accident, which shapes his fear of settling down. The town festival brings them close, but just as he feels ready to commit, the judge orders his release from community service, prompting Ben to leave town and reconsider his life choices.

Before he departs, Lou expresses a desire to spend the night with him, but Ben hesitates, realizing he wants something more meaningful. A chance encounter reveals a laboring patient needing assistance, leading to another diversion when his car is severely damaged during the emergency.

In a twist of fate, Lou, believing that Ben doesn’t want the rustic lifestyle, informs him about plans to marry Hank and arranges for a flight to whisk him away. Torn, Ben bids farewell to his unexpected friends in Grady, feeling unfulfilled despite landing a prestigious job with George Hamilton in California.

As weeks pass, Ben grows disillusioned with the shallow culture of Beverly Hills and the emptiness of his new life. An unexpected visit from Nancy Lee and Hank shakes him further as they affirm their new relationship. Recognizing that true happiness lies back in Grady, Ben decides to return, hoping to rekindle his connection with Lou and find the fulfillment he had long sought.
