As the dark clouds of chaos descend upon the Casa de la Loco Sanitarium, a sinister figure emerges, overpowering a nurse and donning her disguise. The Thief, now camouflaged as a medical professional, embarks on a spree of crime, leaving a trail of destruction in his wake. He pilfers a firearm, commandeers a vehicle, and accumulates a small fortune in stolen cash, all while dispatching one unfortunate loan office manager.

As he flees the scene of the murder, The Thief stumbles upon a morbidly fascinating cemetery, where a somber burial is underway amidst the haunting wails of bagpipe music. A coterie of peculiar mourners, including an undertaker and caretaker, assemble to pay their respects, as if drawn by some macabre curiosity. As the service concludes, The Thief, driven by desperation and avarice, peers into the open coffin, mistaking it for a refuge from his troubles.

However, his decision proves calamitous, as he unwittingly strikes the tuning fork buried with the skeletal remains, unleashing an unsettling cacophony that sends him into convulsions. The lid slams shut, entombing his ill-gotten gains, and The Thief finds himself tumbling into a freshly dug grave, knocking himself senseless in the process.

Upon regaining consciousness, he sets out on a perilous quest to track down every individual who participated in the bizarre funeral rites, driven by an insatiable hunger for retribution. As he stalks his prey, he shows no mercy to those who dare stand in his way, leaving a trail of carnage and despair in his wake.
