Fair warning: we should have seen the shady circumstances coming when a disclaimer stated that no animals were harmed during this episode’s production.

Previously on… Cristina and Major McHottie shared a kiss, while Meredith delved into her mother’s diary. Lexie grew frustrated with George, who failed to notice her or include her among his interns. As Callie and Hahn attempted to navigate their relationship at a slow pace, they unexpectedly found themselves getting off to an exuberant start.

Opening up the past: While sorting through her mother’s belongings for storage in the basement, Meredith stumbled upon old memories. Izzie and Alex spent the night together, prompting Alex to question their exclusivity and whether Izzie would seek comfort with O’Malley again, which she found upsetting. Callie and Hahn celebrated the very nature of their recent intimacy. Hahn expressed that Callie’s moment of clarity reminiscent of her childhood eye exam, when she realized she needed glasses to see the intricacies of nature for the first time. Tears flowed as Hahn had this epiphany, leading Callie to follow suit metaphorically, making her exit.

The Chief introduced Major Owen Hunt as the new head of trauma to Derek and Sloan, announcing a soon-to-be upgrade of the hospital to a Level 1 trauma center by the month’s end. This was crucial, as the absence of Level 1 traumas would effectively reduce the hospital to “Private Practice.” Upon seeing Major Hunt, Cristina quickly left the scene.

The Chief prompted Bailey to embrace her potential as the best surgeon and tasked her with devising a plan to save a young girl with an inoperable tumor, inviting Meredith to collaborate. Meanwhile, the discord between Alex and Izzie resurfaced over their relationship status, with Alex demanding to know if Izzie was wholly committed to him.

Hunt began an intense training session for the doctors—residents and interns alike—using live pigs to simulate trauma scenarios. He thrust a knife into each of the pigs to drive home the imperative: “Save their lives.” The dramatic approach ignited shock amongst the doctors, as they realized the pigs were sedated and felt no pain. Izzie dubbed Hunt “a monster” and stormed off in protest.

Meredith attempted to explain the surgery for the girl with the tumor, utilizing her “Anatomy Jane” doll, but her efforts were sidetracked when the girl’s grandmother insisted on Hahn joining the surgical team. As Callie sought to test a theory about their sexual chemistry with Sloan, Hunt singled out Cristina during the pig experiment, suggesting George outperformed her in the training.

Unanimously, Izzie confronted Derek regarding Hunt’s extreme methods; however, Derek advised her to adapt. Callie, adamantly seeking validation in her relationship with Sloan, fought against her emerging feelings. The day was not without its challenges, as Hunt dispatched most of the doctors to tackle a real trauma, leaving Cristina in charge of saving the pigs. Hahn’s doubts about the girl’s surgical case compounded under pressure, leading to contention with Bailey, who advocated for their risky approach.

Meanwhile, Meredith’s innovative thinking began to make waves as she connected the dots for the case at hand, leading to a highly unorthodox yet plausible solution involving the surgical removal of the girl’s organs. The Chief’s reluctance was palpable, but he eventually acknowledged the strategy’s merit.

As the chaos of surgery unfolded, the frantic tempo of their work mirrored the emotional burdens they carried. In tandem, Izzie and Hunt had yet another confrontation about his unorthodox methods while Cristina and Lexie worked feverishly to save Wilbur the pig. Audiences watched fervently as the tension mounted, with both the girl and the pig seeming to teeter on the brink of disaster.

The narrative interwove relationships and professional dilemmas, revealing the emotional landscape each character navigated. As friends turned rivals in the OR, they ultimately pulled together, successfully saving both patients despite overwhelming odds.

Through moments of vulnerability, such as Callie’s confession and Meredith’s poignant conversation with the Chief about their troubled past, the episode culminated in deeper reflections on their experiences and choices. Cristina eventually faced the harsh reality of their pig patients, leading to a moment of harsh resolve.

As Alex sought clarity in his relationship with Izzie, the episode concluded with lingering questions and emotional resonance as the team members faced both personal and professional uncertainty, alongside moments of unexpected connection and understanding.
