Cassie and Sean, along with her ex-boyfriend Matt and a close friend named Annabel, decide to visit a nightclub housed in an old church, which presents a stark contrast to Cassie’s own religious beliefs. At the club, Cassie encounters two sinister figures: Deathmask, a man donning a clear, plastic mask, and Hideous Dancer, a menacing presence with a scarred face. When Deathmask makes an unwanted advance on her while dancing, Cassie firmly rejects him and steps outside for a moment of respite with Sean.
Meanwhile, in the parking lot, unbeknownst to them, Matt listens in on their conversation. Sean openly declares his feelings for Cassie, and she reciprocates his affection. However, when they return inside, Matt persuades Cassie to share a final ‘goodbye’ kiss. Sean witnesses this moment, and his response is marked by a withdrawal that leads to palpable tension during their drive home. Distracted by her emotions, Cassie struggles to maintain focus on the road, which culminates in a tragic car crash. Cassie’s next recollection is of being rushed to the hospital, where she learns of the devastating loss: Sean was killed instantly in the accident, while both Matt and Annabel emerge physically unscathed.
As the school year unfolds, Cassie is plagued by intense visions of Sean, alongside haunting images of Deathmask and Hideous Dancer intertwined with Matt and Annabel. These visions frequently manifest as nightmarish chases, and though her friends reassure her that these experiences are merely products of her imagination, Cassie’s reality becomes increasingly unstable. After one particularly harrowing chase, she faints and is rescued by Father Jude, a compassionate young priest who offers her a listening ear whenever needed.
A few nights later, feeling threatened once again, Cassie seeks refuge at the church, where Father Jude extends his protection. He gifts her an amulet depicting St. Jude and allows her to rest in his modest quarters. Upon waking, Cassie is struck by a calendar indicating the year 1981. When she inquires about Father Jude, she receives the shocking news of his passing in that very year.
Later, during a swim competition, Cassie finds herself pursued by Deathmask. In a desperate act of self-defense, she stabs him with a fluorescent lamp tube. Yet, when she returns with Matt to investigate, no trace of his body is found—fueling her paranoia that Matt and Annabel are manipulating her with the evil duo. Consequently, she persuades Matt to drive her home, but he instead takes her back to the club to rendezvous with Annabel. Lost in a sea of confusion, she ultimately discovers Annabel with a new beau, Raven. Against the backdrop of danger, Raven warns Cassie to “leave or die,” prompting her departure from the club to the parking lot.
In a moment of rage, Matt drunkenly demands another goodbye kiss, leading to Cassie striking him with a bottle and abandoning him. As she arrives at the site of the horrific accident, she witnesses Annabel’s tragic death, an event that shatters her further. After a near-fatal encounter with a car, Father Jude once again appears, offering his assistance. When she accepts it, she awakens on a hospital gurney next to Raven, who dies soon after whispering words of solace.
Father Jude reveals that her entire ordeal has been a vivid coma dream—the outcome of the accident was in fact that Cassie and Sean survived while Matt and Annabel perished, along with Raven, Deathmask, and Hideous Dancer. Cassie had been traversing an astral plane where the souls of the deceased sought to keep her with them. In this divine narrative, Father Jude is revealed as an angel, and her visions of Sean were what sustained her life. He then poses a crucial question: would she be willing to sacrifice herself to save Sean? Cassie agrees, but when asked if she would want to live for him, she hesitates, stating that she wishes to live.
She then awakens in a hospital room with her loving parents and Sean, marking an emotional reunion filled with relief and joy. In her newfound reality, they celebrate a future together as she prepares to marry the man she loves. There’s also an alternate ending where Cassie jolts awake with Sean beside her, prompting him to inquire about the recurring dream, to which she simply nods in recognition.
