Elisa hesitantly decides to arrange a rideshare via an app to reach her parents’ home in Calabria for an abortion appointment. Climbing aboard an RV helmed by travel blogger Fabrizio, she is joined by a diverse group that includes Riccardo, a doctor, and an outgoing couple, Sofia and Mark. Amidst their journey, Elisa’s morning sickness prompts an emergency stop, after which Mark takes over the driving from Fabrizio. The atmosphere becomes tense when Mark, while driving under the influence of beer, veers off the path, leading to a disastrous crash with a tree as he tries to avoid hitting a dead goat. When Elisa awakens the next morning, she finds the group frantically trying to tend to Mark, who has sustained a severe leg injury. They soon realize they are lost, surrounded by dense forest rather than open road.

After stumbling upon an old cabin, the group’s curiosity is piqued, yet fear keeps them outside. Fabrizio and Riccardo uncover three grotesque scarecrows adorned with pig heads, set up as a grim altar, and decide against entering the cottage. However, driven by a mix of curiosity and dread, Elisa steps inside, where she finds a disturbing mural illustrating three grim deities named Osso, Mastosso, and Carcagnosso, who are worshipped for their promises of wealth in return for human sacrifices. That night, the group opts to stay in the RV, but their peace is shattered by bloodcurdling screams from the house.

Only Mark remains in the RV as the others venture inside and discover a young girl trapped in a hay cocoon, speechlessly terrified as her tongue has been cruelly mutilated. Their horror escalates when they witness masked figures drag Mark into the cabin, binding him to a table while torturing him. They hide in the attic, forced to endure the gruesome spectacle as Mark’s life is snatched away. In the aftermath, the group attempts to escape through the woods, only to discover a graveyard of discarded vehicles, realizing they are but recent victims of these cultists. The frightened girl, later named Chiara, shares her tragic story in a diary, revealing that she is the last surviving member of her family.

As they traverse the same paths repeatedly, the weight of their predicament sets in, leading the group to divulge their personal stories over the dwindling alcohol supply. When night fully descends, Elisa awakens to a terrifying sight: cult members have taken Sofia, Chiara, and Riccardo hostage, preparing them for a sacrificial ritual linked to the ominous deities. In a horrific act, Sofia and Riccardo are mutilated while Elisa and Fabrizio are forced to watch helplessly. The tension escalates as Elisa confronts Fabrizio, realizing he had slyly drugged their drinks and is somehow connected to a larger scheme, revealing a hidden earpiece. As chaos ensues, she is captured and dragged away.

The following morning, Elisa finds herself at a lavish table, her hands pinned to the armrests of a wheelchair during one of the cult’s gruesome banquets. She soon discovers a control room, full of cameras documenting their ordeal, where Fabrizio unveils his shocking role as the director of a twisted amateur snuff film, aiming to craft a horror classic. After a daring escape, Elisa navigates through the film crew’s encampment, witnessing Chiara alive, now an actress and sister to the deranged director Fabrizio. Fatefully, Elisa shoots Chiara in a moment of desperation. Confronting Fabrizio next, she fires at him, capturing their final moments on a handheld camera. As she stumbles upon an escape route labeled falsely as a military zone, she emerges, bloodied yet victorious, onto a beach, where unsuspecting bathers catch her harrowing tale on their phones as she plunges into the sea.

In a chilling post-credits scene, a viewer on the BloodFlix app dissects the very movie that documented her survival, offering a grim critique of Fabrizio’s film and its grotesque nature.
