In the heart of Brooklyn's Dyker Heights, Detective Gino Felino (Gino) - a man deeply rooted in his community - finds himself entangled in a web of chaos when he intervenes in a brutal pimping incident. His partnership with Bobby Lupo is suddenly shattered when Richie Madano, a childhood friend turned crack addict and homicidal maniac, brutally murders Bobby in broad daylight. The streets are stained by the innocent blood of Bobby's wife Laurie and their two children as Richie's psychosis and drug-fueled rage continue to escalate.

Richie's descent into madness knows no bounds as he claims another victim - a random woman shot at a traffic stop for not moving her car fast enough. His goons, though horrified by his actions, remain loyal, allowing him to roam freely through the streets of Brooklyn. Gino, now driven by vengeance and a sense of justice, receives clearance from Captain Ronnie Donziger to launch a manhunt, armed with a shotgun and an unmarked vehicle.

As he navigates the complex web of Richie's associates, Gino visits his mob connections - Frankie and Don Vittorio - who view Richie as a loose cannon that needs to be taken care of. Meanwhile, while cruising through the streets, Gino stumbles upon a discarded German Shepherd puppy and decides to rescue it.

Gino's pursuit of Richie leads him to a bar run by Richie's brother Vinnie Madano, where he faces a wall of silence from the patrons. Failing to extract any useful information, Gino resorts to physical force, dispensing his share of beatings to the recalcitrant crowd. Though he fails to uncover Richie's whereabouts, Gino does manage to quell his own growing attitude problem.

With Pattie Madano, Richie's sister, already in custody, and Richie's estranged father refusing to cooperate, Gino's search for Richie continues with renewed intensity, fueled by a deep-seated need for justice.

As Gino and his wife Vicky, whose marriage is on shaky ground amidst their pending divorce, attempt to rekindle their relationship, they're confronted with a violent home invasion orchestrated by Richie's henchmen. With his son Tony caught in the crossfire, Gino valiantly takes down each attacker, ultimately emerging victorious but shaken. Richie soon exacts his wrath upon Vinnie, brutalizing him for not taking out Gino during their earlier showdown at the bar. The mob receives intel that Gino is holed up there and sends hitmen to finish the job; however, Richie has a surprise waiting – he ambushes them in a fierce gun battle.

Gino's quest for truth takes him to various local hotspots, seeking information about Richie's motives. His investigation leads him to uncover a web of deceit involving Bobby's murder, which was orchestrated by Richie due to his own girlfriend Roxanne Ford's infidelity with the late officer. The discovery of Roxanne's lifeless body at her home only solidifies Gino's suspicions that Richie is responsible for her demise as well. With Laurie, Bobby's widow, he shares his findings and stumbles upon a crucial piece of evidence – the photograph Richie carelessly discarded on Bobby's body after the murder. It transpires that Bobby was a corrupt cop who had been embezzling funds and sought to lead a life of luxury, much like Richie. Laurie, aware of Bobby's illicit activities, had discovered compromising photos of her husband with Roxanne and gave them to Richie as a means of exacting revenge on her cheating spouse.

Gino's pursuit of justice receives a boost when he follows a tip from his informant Picolino, leading him to Richie's hideout in the old neighborhood. With calculated precision, Gino dispatches or wounds each of Richie's henchmen before finally confronting the cunning mobster himself. A brutal hand-to-hand combat ensues, culminating in Gino delivering a fatal blow with a corkscrew to Richie's forehead. The mob arrives soon after, intent on finishing off Richie; Gino, however, takes matters into his own hands and uses the lead mobster's gun to riddle the already-lifeless Richie with bullets. With a twisted sense of satisfaction, Gino instructs the hitman to take credit for Richie's demise when he returns to his boss.
