Eight years have elapsed since the thrilling events of Ek Tha Tiger, and the enigmatic duo of Tiger (Salman Khan) and Zoya (Katrina Kaif) has vanished from public view. Meanwhile, in Ikrit, Syria, a dire situation unfolds as 40 nurses - 25 Indian and 15 Pakistani - are taken hostage by Abu Usman, the ruthless leader of ISC, a terrorist organization. The chairman of the Research and Analysis Wing (RAW), Shenoy (Girish Karnad), recognizes that only Tiger possesses the skills to rescue these innocent lives and that his intervention must occur within a seven-day window, as the United States is poised to launch an airstrike aimed at neutralizing Abu Usman (Sajjad Delafrooz) - who has recently executed a US CIA operative and emerged unscathed from an Iraqi army counterattack. The nurses are being held captive in their own hospital, where Abu Usman lies wounded in the chest.

In the picturesque town of Innsbruck, Austria, Tiger and Zoya now live with their young son, Junior, blissfully unaware of the impending danger. Shenoy and his deputy, Karan, manage to track down Tiger and persuade him to embark on a perilous mission. Although initially hesitant, Tiger ultimately agrees after Zoya implores him to answer his country’s call. Tiger and three other RAW agents - Azaan (Paresh Pahuja), Namit (Angad Bedi), and Rakesh (Kumud Mishra) - set out for Syria.

Shenoy requests Karan to keep a watchful eye on Tiger, fearing that eight years of proximity to an enemy agent may have sullied his character. Tiger’s team infiltrates Iraq via a convoy of oil tankers crossing the Turkish border. Their plan is to pose as illegal immigrants and gain access to the ISC refinery in Ikrit, where they will stage a controlled blast and then feign burns, forcing their captors to hospitalize them.

As they settle into the refinery, they encounter Firdauz aka Tohbaan (Paresh Rawal), an unscrupulous supplier of illegal immigrants who is skeptical of Tiger’s intentions. Meanwhile, Tiger establishes contact with a local source in Ikrit to procure materials for their explosive device. During this encounter, he intervenes to defuse a bomb carried by a young suicide bomber from ISC and dispatches the terrorist’s pursuers, demonstrating his unwavering commitment to justice.

As the situation unfolds, Zoya (character) arrives at the scene where Pakistani nurses are being held hostage alongside Indian nationals. She joins forces with Tiger, accompanied by two other ISI members, Abrar and Javed, in a daring bid to escape the clutches of their ruthless captors. With her timely intervention, they manage to evade their would-be executioners, paving the way for an unexpected alliance between Zoya and Tiger. However, this newfound partnership is met with skepticism from Karan and Shenoy, who harbor reservations about the duo’s joint venture.

The subsequent phase of the operation takes a dramatic turn when a carefully planned controlled blast goes awry, resulting in the unfortunate demise of one security personnel. The refinery owner, outraged by the incident, demands retribution against Tiger and his team. Firdauz, however, intervenes on their behalf, persuading the Emir that the explosion was merely an accident and pleading for clemency. As Firdauz escorts them to the hospital for treatment, Zoya makes a surprise appearance, rescuing Tiger and his companions from further harm.

Firdauz’s true identity as a RAW agent is revealed as he pledges to aid Zoya and Tiger in their mission. While Firdauz tends to the wounded men at the hospital, Zoya sets out to procure crucial plans for the facility. In a shocking discovery, Firdauz uncovers that since the US bombing of Mosul, ISC has transformed the hospital into its primary stronghold in the region, boasting an impenetrable security detail comprising 33 soldiers.

The Indian Intelligence agency becomes increasingly convinced that Usman is plotting something monumental within the hospital’s walls. Shenoy convinces the Indian Prime Minister to deploy defense forces in the Gulf and Iraq, targeting ISC strongholds in Ikrit and Syria. Meanwhile, at the hospital, a conscious Usman forges an unlikely bond with Indian nurse Poorna (Anupriya Goenka). However, when her friends attempt to escape captivity, he brutally eliminates them.

In a separate development, Zoya infiltrates the municipal council building to procure vital information about the hospital’s layout. With calculated precision, she dispatches several ISC operatives and captures the entire ordeal on CCTV footage, which later finds its way to Usman’s attention. The latter orders Firdauz to locate Zoya, setting the stage for a thrilling showdown between these disparate forces.

As Firdauz and Zoya (together) persuade the local US garrison commander to launch an assault on the Ikrit outpost, simultaneously, Tiger and Rakesh concoct a plan within the hospital walls. Under the guise of medical aid, they quietly poison the food supply, ensuring that half the soldiers fall gravely ill by the time the US team attacks. The remaining personnel, oblivious to the treachery, are distracted by the chaos at the hospital, providing Tiger with the perfect opportunity to strike from within and make his escape with a group of unsuspecting nurses.

As he flees the hospital, ISC trucks begin to converge on the scene, signaling that Zoya has fallen into enemy hands - specifically, Usman’s clutches. The cunning terrorist reveals that he was privy to the impending US airstrike all along, and had planned to utilize the ensuing chaos as cover for his own nefarious ambitions in the global terror underworld.

Undeterred by this setback, Tiger launches a counterattack against the ISC forces within the hospital, systematically eliminating each soldier. However, Usman still manages to secure Zoya, chaining her within the building mere minutes before the airstrike is set to strike. Tiger dispatches the nurses to safety with Karan, then turns his attention to Usman, ultimately defeating him and taking his place by Zoya’s side as the clock ticks down.

In a stunning turn of events, Tiger manages to extricate himself and Zoya from the hospital just in time, and the rescued nurses are safely repatriated to India. A full year later, Shenoy receives a call from Tiger, informing him that both he and Zoya have survived their ordeal and are now hiding out in Greece.
