On the eve of their wedding, Zach McCall and his fiancé, Sam, begin a video diary to capture their family history for their future child. After their wedding, they travel to the Dominican Republic for their honeymoon, soaking in the local colors of Carnival and life on the island. A palm reader they meet there unsettles them with a chilling warning: Sam was “born from death” and “they have been waiting for her.” The encounter leaves the couple shaken, and they slip away with a cautious sense of foreboding.

They soon find themselves lost on an unfamiliar street and unable to return to their hotel. A taxi driver named Roger Payano offers to take them to a nightclub, and despite their hesitations they agree. After a night of drinking and dancing, the pair fall unconscious. When they awaken the next day, Sam has been taken to an underground chamber and subjected to a disturbing, unseen ritual.

Back in their hotel, the couple discovers they have no memory of the events from the previous night. A few weeks into their honeymoon, Sam unexpectedly becomes pregnant despite insisting she has been taking birth control religiously since the wedding. The news fills them with joy, and they eagerly share the pregnancy with family and friends, treasuring the idea of a new life.

During the first ultrasound, the baby appears healthy and due date is set for the end of March. Yet the screen flickers with static, briefly cutting to black before returning to normal. As the months progress, Sam’s pregnancy brings a troubling string of symptoms: nosebleeds, mysterious bruising, cravings for raw meat despite being vegetarian, bursts of superhuman strength, telekinetic abilities, and violent outbursts whenever the baby or Sam feels threatened. The couple also notices suspicious watchers lingering around their home, heightening their fear that something dangerous is developing.

A second doctor arrives and performs an amniocentesis, yet he cannot explain what happened to the original physician. Zach opens up about Sam’s past: her parents were killed in a car accident when she was in utero, and she was delivered by cesarean. Before they can settle back into a sense of normalcy, masked men begin secretly installing cameras to monitor Sam’s progress and ensure the baby remains unharmed.

As Sam nears eight months pregnant, the couple attends a church holy communion. Their usually friendly niece (Madison Wolfe) seems unusually frightened of Sam, and the priest who presided at Zach and Sam’s wedding looks at Sam with a disturbing, blood-filled cough. Later, Zach reviews footage from the communion and spots the cab driver in a pew, while the footage from their nightclub night reveals strange symbols on doors. The hospital doctor later explains that the symbol is linked to summoning the Antichrist.

Another thread follows three friends who wander into the woods and encounter slaughtered deer. A shocking moment shows Sam mindlessly consuming the deer’s intestines, and when the friends try to speak to her, she kills them with telekinetic force.

Determined to understand what’s happening, Zach digs deeper and asks his sister Suzie to stay with Sam. He breaks into an apparently abandoned house at the end of his street and discovers CCTV footage of his own home and the missing ultrasound. He is nearly caught by inhabitants who appear to be performing a ritual. In a tense escape, he glimpses the second doctor, the cult leader, and the cab driver among the worshipers.

Returning home, Zach finds their house surrounded by masked men. Inside, Suzie is dead, and Sam screams as a hidden force within her fights to break free. He finds Sam in the baby’s nursery, standing in a trance-like state with a knife against her stomach. He pleads for her to stop, but she presses the knife inward, and a violent blast of light erupts.

The aftermath reveals Sam dead, and the doctor and the cab driver appear as the police interrogate him about his wife’s death, Suzie’s demise, and the disappearance of their child. The final image suggests the nightmare will continue: a honeymooning couple in Paris is offered a lift by the same cab driver, implying that the cycle of danger and ritual will begin again with the next victim.
