As John Hatcher (played by actor) and his partner embark on a relentless pursuit of Hector, their chase leads them down a treacherous path. The stakes are heightened when Hector's betrayal is revealed, culminating in the senseless murder of John's partner at a drug deal gone awry. Reeling from the loss, John hangs up his DEA badge for good and retreats to the comfort of his own home.

However, the allure of his troubled past proves too great, as John finds himself drawn into the midst of a new crisis when he visits old friend Max Keller (actor), only to discover a group of menacing Jamaican drug dealers - known as Posse - lurking around the school where Max works. The tension builds as they cross paths again at a local watering hole, culminating in a violent shootout that leaves John no choice but to intervene and assist in apprehending one of the perpetrators.

As the authorities arrive on the scene, an FBI agent approaches John with a proposition: join the case against the Jamaican cartel. But John's wounds are still fresh, and he declines their offer. Meanwhile, a keen-eyed detective uncovers a symbol etched into the crime scene - a religious icon that serves as a morbid calling card left by the notorious Screwface.

As John delves deeper into the mystery, he finds himself targeted by Screwface's henchmen, who launch a drive-by attack on his sister's home, leaving his niece Tracey (actor) gravely injured. With his family in peril, John interrogates Jimmy Fingers, a shady character from the nightclub shootout, but the gangster remains tight-lipped, forcing John to take drastic measures.

A cryptic clue from one of Screwface's minions only adds fuel to the fire as John becomes increasingly entangled in the web of deceit. His sister's home is soon beset by yet another attack, with the ritualistic sacrifice of her own flesh and blood mere moments away. John arrives just in time to foil the attempt, but not before Screwface himself makes his escape.

Desperate for answers, John enlists Max's help to track down Screwface's gang and uncover the truth behind the mysterious symbols that seem to haunt him at every turn. A high-stakes chase through a posh shopping district ensues, with John and Max taking down several of Screwface's henchmen in a brutal and unforgiving battle. Though they fail to glean any meaningful intelligence from their adversaries, John remains undeterred.

The stakes escalate yet further as Screwface exacts his revenge on John, trapping him between two construction vehicles and setting fire to his vehicle with a Molotov cocktail. But John's refusal to back down proves futile, as he narrowly escapes the inferno, forever changed by the trauma he has endured.

As Jamaican detective Charles Marks (Charles Marks) - a seasoned professional working alongside the FBI to bring down the notorious Screwface - extends an olive branch to John and Max, the trio embarks on a perilous journey to Jamaica, where rumors suggest Screwface has fled. With the aid of a former flame who once shared a personal connection with Charles' past, they manage to procure crucial intel: she reveals that Screwface boasts not one, but two heads and four eyes - an unsettling revelation that sets their sights on his Jamaican stronghold. Under the cover of darkness, the three operatives launch a daring assault on Screwface's heavily guarded complex during a raucous party. John employs an explosive device to divert the guards' attention and shut down the power grid, allowing Max and Charles to unleash a hail of gunfire against the Posse gang as they descend upon the stronghold. Meanwhile, John infiltrates the high-stakes gathering, only to be captured and prepared for a gruesome ritual sacrifice at Screwface's hands. However, John refuses to go quietly into the night, managing to break free from his bonds and engage Screwface in a fierce swordfight that ultimately ends with Screwface's decapitation.

With their initial objective achieved, John, Max, and what remains of Charles' team return to Chicago, where they present Screwface's severed head and prized sword to the remaining loyal gang members in a desperate bid to extort their surrender. But just as it seems like the tide has turned in their favor, Screwface reappears, his unexpected reappearance sparking a violent gunfight that ultimately claims Charles' life. In the aftermath of the carnage, Max provides cover for John's pursuit of Screwface, who is eventually revealed to be the twin brother responsible for the Posse gang's heinous crimes across the United States. After a grueling and intense battle, John finally gains the upper hand, blinding and dispatching Screwface down an elevator shaft, where his lifeless body becomes impaled on a spike, a gruesome spectacle that serves as a stark reminder of the true extent of his depravity. As John and Max depart the crime scene with Charles' lifeless form in tow, they are left to grapple with the harsh realities of their mission and the devastating human cost of their quest for justice.
