The story unfolds ten years after the catastrophic events of Return of the Living Dead 3, focusing on Charles Garrison, who embarks on a dangerous quest to Chernobyl, Ukraine—the site of the 1986 nuclear disaster. His mission? To retrieve the last six canisters of Trioxin 5, an extremely potent and lethal substance. Upon his arrival, he encounters two Russian sellers who direct him to an abandoned power station rumored to house the precious Trioxin. However, the situation escalates when one of the Russians inadvertently exposes himself to the toxic gas, turning him into a reanimated corpse. With no other option available, Charles is forced to take action, swiftly eliminating the zombie with a shot to the head.

As the fallout from this chaotic event settles, we learn that Charles has taken in his nephews, Julian and Jake “Pyro” Garrison, ever since their parents, who were volunteers, tragically passed away during an incident at Hybra Tech.
