The curtain opens on Nathan Drake (Tom Holland), a daredevil with a penchant for danger, dangling precariously outside an airborne plane as cargo falls around him like confetti. Undeterred by the mayhem unfolding beneath his feet, Nate attempts to scale the aircraft's hull, evading henchmen and debris with the agility of a cat in a sack factory. Just as he nears the top, a runaway vehicle from the plane's ramp hurtles towards him, sending him tumbling through space once more. Fate intervenes, however, as an unseen hand grasps for him, arresting his fall.

Fast-forward 15 years to Boston, where youthful Nathan (Tiernan Jones) and his older brother Sam (Rudy Pankow) are hatching a plan to pilfer the original map of Ferdinand Magellan's expedition from a museum. The legend surrounding Magellan's journey whispers tales of hidden riches, and the brothers are determined to claim their share of this treasure. Their nocturnal escapade is short-lived, as they're apprehended by security and Sam faces the harsh reality of being expelled from their orphanage home.

As Nate consoles his departing sibling in their shared quarters, Sam passes down a family heirloom - a ring bearing the inscription "Sic Parvis Magna" ("Greatness from small beginnings") - a reminder to his little brother to never forget their ancestral legacy. Sam vanishes into the night, leaving Nate with a promise to return and a poignant reminder of their bond.

His chance encounter with a haughty socialite provides the perfect opportunity for him to swipe her prized bracelet, a move that catches the eye of Victor "Sully" Sullivan (Mark Wahlberg), a seasoned fortune hunter with a reputation for getting his hands dirty.

As they prepare for the heist, Nate takes it upon himself to conduct thorough background checks on Sully, seeking to understand the true nature of their partnership. Meanwhile, the duo sets its sights on the auction, where Sully's instincts scream warning as he spots Jo Braddock (Tati Gabrielle), a notorious mercenary with whom he has had previous run-ins.

As tensions rise, Nate readies himself to disrupt the power supply, but is caught off guard by Moncada's burly Scottish henchman (Steven Waddington). Sully must think on his feet, employing his cunning and charisma to outbid Moncada, despite knowing he lacks the financial means. After evading their pursuers, Nate creates a diversion by dangling precariously from the chandelier, drawing attention away from Sully as he snatches the cross.

Their daring escape is nearly foiled when Braddock reappears, her intentions far from pure. Sully and Nate manage to slip away, but not before she dispatches their security detail with ruthless efficiency. In Barcelona, they meet up with Sully's associate, Chloe Frazer (Sophia Ali), who possesses the second cross. Though initially wary of Nate's involvement with Sully, Chloe's own motives are soon revealed to be less than altruistic when she pilfers the cross from Nate's bag. The ensuing chase sees Nate and Sully hot on Chloe's heels, but their quarry remains one step ahead. As they regroup, Nate reveals that Chloe doesn't actually possess the knowledge necessary to locate the treasure without their assistance.

In a desperate bid to uncover the secrets of Santa Maria del Pi, Nate pores over the diary's cryptic clues, piecing together the mystery. Meanwhile, Chloe drops a bombshell that Sully has been keeping from Nate – a revelation that threatens to upend their partnership and jeopardize their quest for Magellan's treasure.

At the Moncada Foundation Headquarters, Moncada's ire is piqued by his father Armando's (Manuel de Blas) decision to relinquish their family fortune. He confides in Santiago that he does not deem himself worthy of inheriting it, and after a final poignant conversation with his father, he instructs Braddock to eliminate Armando before making off with the funds to pursue his own expedition.

Meanwhile, Nate, Sully, and Chloe venture to the church, where they stumble upon a hidden passageway beneath their feet. Utilizing the cryptic clues left behind by the diary, they attempt to unlock an entrance, but Sully's misinterpretation of the instructions inadvertently triggers a booby trap. Nate and Chloe descend into the depths with one key while leaving the other with Sully above ground.

As they navigate the underground labyrinth, Chloe reveals her deep-seated mistrust of people, stemming from her father's abandonment of his family after exploiting a discovery she made. The trio eventually finds themselves in a seedy nightclub, where they must outsmart the Scotsman and another henchman sent by Braddock to retrieve them.

As they make their escape, Nate and Chloe discover that their path is fraught with peril, as Sully becomes embroiled in a cat-and-mouse game with Braddock herself. Despite being ambushed, Sully manages to fend off his attackers, using the second cross to unlock a hidden entrance behind the glass wall of a restaurant. His timely intervention saves Nate and Chloe from drowning as the room fills with water.

The trio emerges into another chamber, where they find two giant urns filled with salt instead of gold. As they begin to frantically search for answers, Sully sacrifices his own cross to help them escape, only to be blindsided by Braddock's treachery. Chloe seizes this opportunity to deceive Nate and pilfer the map, revealing that the treasure lies in the Philippines.

In a chilling turn of events, Chloe brutally pistol-whips Nate, knocking him unconscious as she exacts her revenge for his perceived deceit. With Sully still reeling from their earlier confrontation, Chloe's betrayal leaves the two men stranded and helpless in the face of her calculated treachery.

As Sully locates Nate, he's met with a somber reality - his friend is consumed by grief and anger, seeking answers about Sam's disappearance. Sully confesses that they were ambushed by Braddock, who callously gunned down Sam, leaving him no choice but to abandon his comrade in the chaos. As they prepare for their expedition to the Philippines, Nate and Sully reconcile and concoct a plan to stowaway on the plane by hiding in the trunk of Moncada's vehicle.

Upon arrival, Moncada gathers his crew and shares his family's tragic history, claiming that Magellan's crew had betrayed them, stealing the gold that rightfully belonged to the Moncadas. But before he can finish his impassioned speech, Braddock betrays him, coldly slitting his throat in front of his stunned followers. Seizing control of the group, Braddock sets his sights on finding the fabled treasure.

Nate and Sully seize their chance to escape, but Nate's pride gets the better of him as he confronts Braddock about shooting Sam. His actions inadvertently trigger a chain reaction that leads Sully to make a daring escape via parachute, while the henchmen are sent plummeting from the plane. Chloe manages to snatch the map and slip away unnoticed, only to be thwarted by Nate's cleverness. As they float towards an island resort on a cargo load with a parachute, they finally land safely.

Once settled in their room, Nate and Chloe attempt to decipher the cryptic map, but it offers no clear direction. That night, while Chloe sleeps, Nate uses one of Sam's postcards as a makeshift cipher, carefully igniting the flame to reveal a hidden message - "The keys are your compass." Employing this newfound insight, Nate locates the exact spot on the map by following a trail of crosses. He deliberately falsifies coordinates, knowing that Chloe will follow them, and she does. As Nate's speedboat approaches the fabled location, it's not long before he crosses paths with Braddock and her cohorts, who have arrived via more conventional means. Sully, ever the opportunist, happens upon Nate and together they stumble upon a cavernous wonderland featuring two pristine ships, their hulls brimming with glittering gold. The treasure trove is too great to ignore, and the duo sets about claiming their share just as Braddock's henchmen begin airlifting the ships themselves, valuing them at an astonishing sum. Sully seizes the opportunity to take down her adversaries from above, dispatching Braddock's goons with ease before turning her attention to the Scottish chopper pilot who finds himself entangled in a desperate dogfight.

Meanwhile, Nate and his new ally are engaged in their own battle royale as they fend off the airborne assault. The Scotsman's aerial antics culminate in a spectacular crash landing on a giant rock face, while Braddock attempts to take matters into her own hands by pursuing Nate with reckless abandon. As he attempts to scramble aboard a departing chopper, Sully is forced to make a difficult decision: save Nate or protect the treasure. In the end, she chooses neither, instead opting to send the gold tumbling towards its rightful owner in a watery grave.

With Braddock crushed beneath the wreckage of one of the ships, Nate and Sully make their escape, with the former revealing he's managed to salvage a small portion of the loot for himself. As they soar into the distance, they take great pleasure in taunting Chloe, who remains oblivious to their scheme on her trusty speedboat below. The aerial duo then sets its sights on their next adventure, leaving the dust to settle on their latest escapade.

As the credits roll, we're treated to a glimpse of Sam's continued survival behind bars, where he's hard at work crafting another postcard for Nate. Meanwhile, midway through the credits, we find Nate conferencing with Gage (Pilou Asbæk) regarding the acquisition of a valuable item for Roman, while Sully sports a dashing new mustache that earns him a good-natured ribbing from his partner in crime. Just as they think they've secured their exit, however, they're ambushed by a group of ruthless gunmen looking to put an end to their high-flying escapades once and for all.
