The film begins with John Kramer, portrayed by Tobin Bell, undergoing an MRI scan at a hospital where a doctor informs him that he has only months to live due to advanced brain cancer. During his stay, John witnesses a custodian, played by Isan Beomhyun Lee (Isan Beomhyun Lee), trying to steal from a patient. In a vivid imagination sequence, the custodian finds himself ensnared in a gruesome trap devised by John, where he has a mere 60 seconds to save himself from losing his eyesight.

Fast forward four months, John, now deeply affected by his diagnosis, meets Henry Kessler, portrayed by Michael Beach, at a cancer support group. Henry claims to have terminal cancer yet remains hopeful about his fight. To John’s surprise, he later encounters Henry at a coffee shop, looking much healthier and claiming to have been cured by an experimental treatment developed by Dr. Finn Pederson (Donagh Gordon). Intrigued, John researches and finds Dr. Pederson’s daughter, Dr. Cecilia Pederson (Synnøve Macody Lund), who reveals that her father is being pursued by corrupt pharmaceutical companies, prompting her to conduct secret clinical trials.

Following Cecilia’s lead, John travels to a makeshift clinic in Mexico City. Upon arrival, he meets Gabriela (Renata Vaca) and her medical team, which includes nurse Valentina (Paulette Hernandez), anesthesiologist Mateo (Octavio Hinojosa), and, most notably, the enigmatic Dr. Cortez. Confident in the efficacy of the treatment, John leaves to buy a congratulatory tequila for Gabriela but upon returning, finds the facility abandoned. The truth dawns on him: the surgery never occurred, and he has been conned.

In desperate retaliation against the deceitful operation, John ensnares Diego, the taxi driver who was part of the scam. Diego is subjected to a brutal test where he must mutilate himself to survive. Meanwhile, the con artists face an even grimmer fate as they are abducted one by one by Amanda Young (Shawnee Smith), John’s apprentice, alongside John himself.

They awaken in a sinister factory, taken captive in a twisted game orchestrated by John and Amanda. Each member of the fraudulent team must confront their worst fears in a series of violent tests that lead to horrific outcomes, from Valentina’s gruesome leg amputation to Mateo’s macabre self-surgery. A struggle ensues when Parker Sears (Steven Brand) enters the scene, guns blazing, demanding a refund, only to be chained up and forced to witness the chaos.

In a shocking turnaround, Cecilia reveals her collusion with Parker, brutally killing Gabriela, further intensifying the conflict. They capture John and expose him to a terrifying contraption designed to drown him in blood while innocent Carlos is shackled opposite him, amplifying John’s torment.

As John fights to protect Carlos, Parker and Cecilia attempt to escape with the proceeds from their scam. But as greed takes the forefront, they trigger lethal traps that ultimately seal their fates. In a harrowing twist, Cecilia betrays Parker, ensuring her survival at the cost of his life and finding herself trapped while John and Carlos break free.

The film culminates with John, Carlos, and Amanda walking into the sunset, leaving behind a path of treachery and despair. In a mid-credits scene, John, alongside Detective Mark Hoffman (Costas Mandylor), prepares to subject Henry to a nightmarish trap, further establishing the cycle of vengeance and survival.
