Marv, portrayed by Mickey Rourke, awakens disoriented on a highway looking over the Projects. Surrounded by multiple deceased young men and a crashed police vehicle, he struggles to piece together the events that led him to this moment. As he retraces his steps, he remembers it’s Saturday night and recalls watching Nancy Callahan dance at Kadie’s Saloon earlier. However, his night takes a dark turn when he encounters a group of wealthy frat boys committing a heinous act against a homeless man. When Marv intervenes, the ringleader shoots him in the arm, calling him “Bernini Boy,” a term Marv misinterprets as “Bernie.”

Determined to seek justice, Marv chases after the fleeing frat boys, stealing a police car in the process. In a reckless move, he crashes the vehicle into theirs, which results in another blackout. As he regains focus, he finds himself in The Projects, his childhood neighborhood, where he unearths the brutal vengeance of the residents lurking in the shadows. With their support, he takes down the remaining frat boys and interrogates the leader about the nickname. Upon learning it refers to the brand of coat he’s wearing, Marv slits the boy’s throat out of frustration, pondering how he came to own the coat he cannot remember acquiring.

In a parallel storyline, Johnny, a brash young gambler, arrives in Sin City with hopes of striking it rich. He makes his way to Kadie’s, where fortune smiles upon him as he wins big at the slot machines. Accompanied by his good luck charm, waitress Marcie, he entices her to join him in a high-stakes poker game against the formidable Senator Roark. Tensions escalate when Johnny decisively wins, infuriating the senator and drawing unwanted attention from corrupt police lieutenant Liebowitz. Instead of heeding the lieutenant’s warning to leave town, Johnny decides to revel in the moment, leading to a violent confrontation with Roark’s henchmen. In a shocking twist, Roark, revealing his kinship with Johnny, exacts brutal retribution by breaking three of his fingers and shooting him before abandoning him alive, ensuring Johnny’s suffering continues.
