Set immediately following the preceding film, Cameron Elam encounters a severely injured Deputy Frank Hawkins on his way home from a Halloween party. Haunted by regret over his role in allowing the notorious serial killer Michael Myers to remain alive, Hawkins vows to take matters into his own hands and eliminate Michael that very night. In a gripping flashback to 1978, shortly after the events of the original movie, Hawkins accidentally shoots his partner during the chase for Michael, a mistake compounded by his intervention that prevented Dr. Sam Loomis from delivering a lethal blow to the killer in front of Michael’s childhood residence.
Fast forward to October 31, 2018, and Tommy Doyle, during the 40th anniversary of Michael’s confinement, is joined by familiar faces, including Marion Chambers, Lindsey Wallace, and Lonnie Elam, Cameron’s father. Meanwhile, following a dangerous encounter with Michael, Laurie Strode, along with her daughter Karen and granddaughter Allyson, watches in horror as a group of firefighters unwittingly frees Michael from the basement of the burning Strode home. With chilling precision, Michael dispatches the firefighters one by one before setting off for Haddonfield.
In the midst of the chaos, Karen and Allyson rush Laurie into emergency surgery while Michael viciously murders an elderly couple in their home. An emergency alert soon informs Tommy, Marion, Lindsey, and Lonnie of Michael’s escape, leading to a disturbing encounter for a bar patron named Vanessa, who believes she spots Michael in her car. This prompts Tommy and his friends to confront the terror but to no avail, as the driver manages to slip away unnoticed.
Determined to protect the town, Tommy rallies a group of enraged Haddonfield citizens to track down Michael before he can inflict more harm. Meanwhile, unaware of the full extent of the danger, Karen chooses to shield Laurie from the news of Michael’s breakout, seeking to allow her time to recover. As the night unfolds, Cameron invites Allyson to join him and Lonnie in their mission to find Michael, igniting a flame of revenge within her.
Reunited in the hospital, Laurie and Hawkins reflect on their past, hinting at Hawkins being the biological father of Karen. In a terrifying sequence, Marion, Lindsey, Vanessa, and her husband Marcus attempt to alert others about Michael and fall victim to their warnings, with only Lindsey managing to survive.
As Tommy transports Lindsey to safety, Lonnie, Cameron, and Allyson chart out Michael’s tragic past and determine that he is headed back to his childhood home. Elsewhere, Tommy reconnects with former sheriff Leigh Brackett, who is still grieving for his daughter, Annie, who was killed in 1978. The stakes rise as Tommy organizes a massive mob of citizens, sharing the dreadful news of Michael’s survival with Laurie, who prepares for a confrontation of her own.
In the meantime, Michael brutally dispatches the unsuspecting current residents of his childhood home while Lance Tovoli, an escaped hospital inmate wrongly believed to be Michael, is chased through the hospital by Tommy’s mob. Despite Karen’s efforts to assist him, Lance makes a desperate leap from a hospital window, resulting in his demise, leaving Karen distraught.
As Laurie’s condition deteriorates, she urges Karen to join Tommy in the hunt for Michael. At Michael’s old home, the tension escalates when Lonnie enters alone, but Allyson and Cameron soon follow, only to discover Lonnie’s lifeless body in the attic. A tragic sequence unfolds as Cameron also meets his end at Michael’s hands, with Allyson suffering a leg injury in the process. In a moment of bravery, Karen confronts Michael, stabbing him with a pitchfork and momentarily gaining the upper hand by removing his mask and taunting him, allowing Allyson to flee.
In a harrowing turn of events, Karen leads Michael straight into Tommy’s vengeful crowd, leading to an apparent victory as they stab him and believe they have vanquished him. However, in a chilling twist, Michael rises again, claiming the lives of several members of the mob, including Tommy, who is brutally killed with his own baseball bat.
As Karen dares to look into the upstairs window of Michael’s childhood home, she recalls the haunting tale of him staring out of it. In a twist of fate, Michael appears behind her, and the screen fades to a nightmarish end, suggesting her impending doom.
