Eleven years have passed since the turmoil in Burma, and John Rambo (Sylvester Stallone) has settled into quiet life on his late father’s horse ranch in Bowie, Arizona. Alongside his close friend, Maria Beltran (Adriana Barraza), and her granddaughter, Gabriela (Yvette Monreal), Rambo has found solace in raising and training horses. Gabriela dreams of going to college and shares with Rambo her desire to find her biological father, Miguel (Rick Zingale), who she recently discovered lives in Mexico. Despite Rambo’s and Maria’s hesitations, arguing that individuals like Miguel never truly change, Gabriela is determined to confront her father about his abandonment. Ignoring their warnings, she is led by a friend, Gizelle (Fenessa Pineda), to Miguel’s apartment, only to be met with cold indifference that shatters her hopes.
In a cruel twist of fate, Gizelle takes Gabriela to a club where she falls victim to a drugging and kidnapping by a ruthless Mexican cartel. Upon learning of Gabriela’s disappearance, Rambo leaps into action, tracking down Miguel and Gizelle to demand answers. It’s through keen observation that he unravels Gabriela’s betrayal, recognizing a bracelet belonging to her mother on Gizelle’s wrist, which seals the fate of both women.
Led by Gizelle, Rambo confronts El Flaco (Pascacio López), the last person to communicate with Gabriela. A mysterious journalist named Carmen Delgado (Paz Vega) trails Rambo as he fights through the cartel’s defenses led by the vicious brothers, Hugo (Sergio Peris-Mencheta) and Victor Martinez (Óscar Jaenada). The confrontation takes a brutal turn when Rambo is marked and left for dead by the cartel, who have sinister plans for Gabriela as punishment for Rambo’s interference.
