Thirty days following the contamination of The Hive, the Umbrella Corporation naively deploys a research team to reopen the facility and investigate the outbreak. The only survivors from the initial incident are Alice (Milla Jovovich) and Matt Addison (Eric Mabius), with Alice having been subjected to experiments and Matt having entered a mysterious “Nemesis Program.” As the team attempts to access the sealed blast doors, they are brutally overrun by a horde of infected creatures.
The chaos spreads beyond the facility, reaching Raccoon City, where the infection infects the general population. In a desperate bid to contain the situation, Umbrella quarantines Raccoon City and establishes a security perimeter. However, the situation escalates when Angela Ashford (Sophie Vavasseur), the daughter of a Level 6 Umbrella researcher Dr. Charles Ashford (Jared Harris)—the very creator of the T-virus—goes missing after an Umbrella security vehicle crashes while attempting to evacuate her from the city.
Awakening in a deserted hospital, Alice discovers the city is in ruins and filled with the infected. Armed with a shotgun retrieved from a police vehicle, she is haunted by visions of a man who experimented on her, leading to her newfound superhuman abilities.
Meanwhile, disgraced police officer Jill Valentine (Sienna Guillory), having been suspended for exposing the truth about the infection, encounters Sergeant Peyton Wells (Razaaq Adoti) at Raven’s Gate Bridge, the sole exit from the city. As the T-virus engulfs the city, a heart attack victim transforms into a zombie, resulting in Peyton getting bitten during the chaos. Fearing the T-virus spreading, Umbrella supervisor Timothy Cain (Thomas Kretschmann) orders his soldiers to seal the gates, forcing the civilians back into danger.
Across town, Carlos Olivera (Oded Fehr) and other soldiers align with surviving S.T.A.R.S. teams to combat the zombie onslaught. Though they manage to save a comrade, the battle takes a dire turn, forcing them to retreat as reinforcements arrive too late. A helicopter they depend on escapes without them, further highlighting the betrayal by Major Cain, who succumbs to paranoia.
In a church, Jill, Peyton, and reporter Terri Morales (Sandrine Holt) barricade themselves against the invaders. In a chilling twist, Jill discovers a priest who has kept his zombified sister alive. As chaos ensues when Lickers invade the church, they are miraculously rescued by Alice, who arrives just in time.
Umbrella’s Nemesis Program is unleashed to exterminate remaining S.T.A.R.S. operatives, resulting in a deadly confrontation. L.J. (Mike Epps), a surviving citizen, manages to evade Nemesis by surrendering his weapons. Meanwhile, Dr. Ashford seeks to keep communicating with Alice, directing her toward Angela’s location in exchange for their evacuation.
As the urgency rises, Alice documents the unfolding horror with Terri’s video camera, all while learning of the impending nuclear strike set to obliterate Raccoon City. When Nemesis cornered them, Alice takes on sheer desperation, trying to stop him with all her might despite his invulnerability. Jill’s fierce determination propels her to eliminate a transformed Peyton, who has succumbed to the virus.
On the quest to uncover Angela, the group encounters a harrowing reality of infected children and dangerous creatures. Tragically, Terri meets her end when surrounded by the undead, prompting Jill to step up when she unearths the critical connection to Angela. Amidst bloodshed and betrayal, Jill, Carlos, and Alice converge in their efforts to secure Angela and themselves.
In a heart-wrenching moment of sacrifice, Nicholai gives his life for Jill and Angela to escape the packed school as Alice uses the last of her strength to fend off more horrors. As the survivors regroup, Angela reveals she has been linked to the very virus they seek to eliminate, a painful personal connection to Dr. Ashford’s intentions.
As the nuke countdown draws closer, the team battles against overwhelming odds—facing relentless undead and betrayal from Cain. In a fight for humanity’s sake against Nemesis, Alice confronts the truth about her past with Matt, reliving moments of lost humanity within the monster.
With the city facing annihilation, a final showdown leads to Alice and the remaining group barely escaping captivity. Yet, they are left with a bitter resolution as they realize the deception surrounding the event—Umbrella spins the narrative, constraining the truth within layers of lies. Just when victory seems in reach, Alice finds herself back in the Umbrella facility where experiments resume, unveiling the broader conspiracy at play.
As her past and future collide, the cycle begins anew, indicating this fight for survival is far from over.
