In a haunting flashback, young Claire and Chris Redfield find themselves at the Raccoon City Orphanage, where Claire forms a bond with Lisa Trevor, a girl who has endured horrific experiments. Their fate intertwines with Dr. William Birkin from the Umbrella Corporation, who oversees the orphanage and is notorious for his cruel experiments on the children. As the experiment unfolds, young Claire narrowly escapes Birkin’s grasp.

Fast forward to 1998, Claire returns to her hometown of Raccoon City, hitching a ride in a semi-truck. An eerie incident occurs when the driver runs over a woman who inexplicably vanishes, leaving behind only a mysterious pool of blood. The truck’s Dobermann soon becomes infected, displaying erratic behavior. Meanwhile, Leon S. Kennedy, a rookie cop, wakes up for his first shift at the Raccoon City Police Department, only to find himself dozing off at a diner, where unsettling signs of a zombie outbreak start to emerge.

Claire makes her way to her brother Chris’s house, picking the lock with her knife. As they talk, Claire tries to warn Chris about the nefarious activities of the Umbrella Corporation, claiming contact with conspiracy theorist Ben Bertolucci, who suspects Umbrella has poisoned the city’s water supply. Just as chaos begins to unfold, a chilling siren warns citizens to stay indoors, prompting Chris to leave for the police station while Claire opts to take his motorbike.

At the police station, the STARS Alpha team gathers with Chief Irons to discuss the alarming events unfolding in the city. Chief Irons voices his confusion over the sudden changes, relaying that the Bravo team sent to investigate the eerie happenings at the Spencer Mansion has gone missing. As they prepare, unknown instructions for Wesker become evident, hinting at deeper, darker dealings with Umbrella.

The tension escalates as the Alpha team flies to the mansion, eventually discovering the remnants of the Bravo team. Divided inside, Wesker and Jill Valentine discover a concealed passageway, while a catastrophic helicopter crash rocks their already perilous situation. Meanwhile, Chris and Richard Aiken confront the gruesome fate of their fallen comrades. With chaos erupting around them, Wesker’s true motivations come to light as he aims to expose Umbrella’s secrets for personal gain.

As night descends, Leon’s tranquil shift crumbles when the truck driver, now a zombie, crashes into the police station. Overwhelmed, Chief Irons puts Leon in command before fleeing the city, where he encounters perilous Umbrella guards. In this chaos, Claire comes to the aid of an attacked Chief, while Leon discovers Ben locked away, only for Ben’s attempt to escape to lead to his unfortunate demise.

The trio of Claire, Leon, and Irons venture to the Orphanage, searching for a secret Umbrella tunnel. Here, they face a terrifying Licker, leading to Irons’ gruesome end. Just when all hope seems lost, Lisa reappears to assist them, providing the keys to escape.

Meanwhile, Wesker stumbles upon Dr. Birkin—who is engrossed in his experimentation. A tense standoff ensues, revealing a deadly showdown where emotions run high as Birkin’s transformation into a grotesque creature begins. The horrific climax reaches its zenith when Claire, Leon, Chris, and Jill unite to escape the oncoming destruction, only to confront the fully mutated Birkin on a train. With a final, explosive confrontation, Leon employs a rocket launcher to bring down the abomination.

In a chilling mid-credits scene, Wesker awakens in darkness, only to find a figure offering him sunglasses, revealing herself as Ada Wong, setting the stage for further mystery ahead.
