The film begins with Alice (Milla Jovovich), the protagonist from the previous installments, awakening in a shower, dazed and terrified. Upon realizing she is back at the Spencer Estate in Raccoon City, where it all began, she struggles to comprehend her situation. After putting on a striking red dress found on the bed, she cautiously steps into the hallway, feeling an intense sensation of being preyed upon. Making her way to the infamous laser room from the original film, she narrowly escapes certain death by leaping into a vent before a terrifying trap activates. She drops into a hospital corridor, where the eerie silence is shattered when a massive guillotine cuts a nearby rolling bed in two. After a grim smile, she manages to evade this blade, only to be gunned down by a second trap that unexpectedly erupts from the floor. As her lifeless body lies there, three scientists from Umbrella Corporation, including Dr. Issacs (Iain Glen), approach and discuss disposing of her. They throw her remains into an outdoor pit littered with other dead clones of Alice. This facility is revealed to be just a small segment of an expansive Umbrella underground base located in the North American West.
From here, the real Alice provides us with a harrowing update, detailing the aftermath of the previous movie, Resident Evil: Apocalypse (2004). She explains that the epidemic caused by the T-Virus quickly escalated beyond the isolation of Raccoon City, infecting the entire North American continent within weeks and spreading globally shortly thereafter. This catastrophic event devastated ecosystems, annihilating nearly all plant life and transforming the world into a desolate wasteland. Humanity was forced into a nomadic lifestyle, constantly searching for refuge and safety.
As we journey with Alice, she rides her motorcycle into the derelict ruins of Salt Lake City, Utah, while listening to a radio broadcast claiming the existence of survivors. Upon stopping outside the station, she unsheathes her twin machetes and cautiously enters. To her dismay, the station is devoid of zombies, signaling the broadcast is likely a trap. Inside, she discovers an elderly woman who mourns her “baby,” only to find a lifeless infant. As she drops the corpse in shock, Alice is ambushed by the station’s five residents, who deceptively lure victims to rob them. Captured and thrown into a cage-lined pit filled with zombie dogs, Alice showcases her agility, defeating two of the beasts before restraining the others. However, their chaotic antics lead to a collapse that sends the station’s inhabitants into the pit, where they fall victim to their own unleashed creatures, granting Alice a quick escape while reclaiming her weapons.
Meanwhile, in Nevada, a substantial convoy equipped with classic rock music traverses the arid landscape. Among the convoy members are former Umbrella soldiers Carlos (Oded Fehr) and LJ (Mike Epps), who share friendly banter over the CB radio regarding cigarettes, revealing a bond built through shared experiences.
In stark contrast, Dr. Issacs faces the ire of his superior, Chairman Albert Wesker (Jason O’Mara), at Umbrella’s North American HQ, where discussions reveal the urgent need for a cure for the T-Virus. Issacs insists that he requires the original Project Alice to develop a solution, stating that the existing clones have proven inadequate. The grim demand from Wesker makes clear the stakes as he coldly declares a one-week deadline.
As fate unfurls, Alice strives to refuel at a deserted gas station only to find no power; she ventures inside and discovers a journal with vital transmission logs hinting at a “safe” location in Alaska. Just as she exits with this vital information, a crow with an eerie eye captures her attention, and she picks up a radio broadcast from Mikey (Christopher Egan) detailing the convoy’s location.
The convoy arrives at a seemingly abandoned motel, verified by Carlos and LJ as safe. However, a close encounter leaves LJ with a bite from a zombie after he investigates a room. He manages to fend off the creature but is marked with a grim souvenir. Tension rises that evening as Claire (Ali Larter) and Carlos converse about their tiresome journey, revealing Claire’s growing exhaustion from their relentless movement. Meanwhile, Alice experiences troubling dreams during an uneasy slumber in the desert, awakening to unleash psionic energy, further complicating her situation.
As night descends, Alice dreams of danger while crows gather around K-Mart (Spencer Locke) and Claire, immediately instigating a panic to evacuate. However, as they flee, chaos breaks out when the crows attack, overwhelming the group. Amidst the pandemonium, Carlos and LJ manage to save the children while Alice exerts her newfound abilities to shield her allies, defeating the crows in a spectacular display of power.
