In this chilling prequel to The Texas Chainsaw Massacre: The Beginning, the narrative unfolds in rural Texas during the year 1955, focusing on the Sawyer family. The story commences at a birthday celebration hosted by the family matriarch, Verna Sawyer, in honor of the family’s youngest member, Jedidiah. As part of his initiation into the family’s brutal traditions, he is given a chainsaw and compelled to inflict pain on an accused thief attempting to steal livestock. Disturbed by the act, Jed refuses, but the man meets a grisly fate at the hands of Grandpa.
Fast forward a few months, and we meet Betty Hartman and Ted Hardesty, a young couple driving through the family’s territory. Their fateful encounter with a seemingly injured Jed leads Betty to a rundown barn where tragedy strikes as she is ambushed by the Sawyer clan. Her father, Sheriff Hartman, later discovers her lifeless body and, seeking vengeance, takes the young Jed into custody and sends him to a mental asylum for troubled youths.
The film shifts to 1965, introducing the new nurse, Lizzy](/actor/vanessa-grasse), who forms an unexpected bond with two psychiatric patients, Bud and Jackson (Sam Strike). However, chaos erupts when Verna bursts in with a court order to free her son. When her request is denied, she resorts to violence, igniting a riot that leaves many staff members and patients dead. In the midst of the turmoil, Lizzy is rescued by Jackson, but they soon find themselves taken hostage by two deranged escapees, Ike (James Bloor) and Clarice (Jessica Madsen). The trio’s nightmare escalates when they stop at a diner, leading to a harrowing massacre.
As the night progresses, Sheriff Hartman, along with Deputy Sorrell, discovers the bloody aftermath of the diner attack and learns that the teenage Jed is among the surviving escapees. Meanwhile, the fugitives seek refuge in an abandoned mobile home where disturbing events unfold, including seduction, brutality, and the shocking death of Ike at Bud’s hands.
The next day brings more peril as Clarice is captured by Sheriff Hartman and defiantly taunts him before facing a tragic end. Meanwhile, Lizzy and Jackson navigate the chaos but suffer a loss when Bud is inadvertently killed, igniting Jackson’s fury as they steal a police car to escape Hartman’s relentless pursuit.
In a dramatic turn of events, Lizzy finds herself trapped in a cop car while communicating with an officer named Sorrell. He reveals the horrifying truth about Hartman having Jed at the site of Betty’s demise. As Verna seeks to rescue her son, revelations unfold, revealing Jackson’s actual identity as Jed. This climactic confrontation leads to a twisted family reunion where Verna, enraged and desperate, confronts Hartman.
Back at the Sawyer household, Verna tends to Jed’s severe injuries while concurrently orchestrating the family’s gruesome ritual. Stirring a heart-wrenching atmosphere, Verna bestows upon Jed his initial chainsaw, forcing him into a horrifying legacy as she urges him to eliminate any threats, claiming that it is an act of loyalty to their family.
